Foreword
ISO (the International Organization for Standardization) is a worldwide federation of national standards bodies
(ISO member bodies). The work of preparing International Standards is normally carried out through ISO
technical committees. Each member body interested in a subject for which a technical committee has been
established has the right to be represented on that committee. International organizations, governmental and
non-governmental, in liaison with ISO, also take part in the work. ISO collaborates closely with the
International Electrotechnical Commission (IEC) on all matters of electrotechnical standardization.

1 Scope
This part of ISO 14243 specifies the relative angular and translational movement between articulating
components, the pattern of the applied force, speed and duration of testing, sample configuration and test
environment to be used for the wear testing of the patello-femoral joint of total knee prostheses.

3 Terms and definitions
For the purposes of this document, the terms and definitions given in ISO 7207-1 and the following apply.
3.1 patello-femoral coordinate system
joint coordinate system used to describe the relative motion and forces applied between the patellar and
femoral components. The system consists of: a Cartesian coordinate system rigidly fixed to the femoral
component comprising of the femoral flexion/extension axis, femoral superior/inferior axis and femoral
anterior/posterior axis; the patellar internal/external rotation axis which is fixed to the patellar component; a
floating axis; and the patellar compression axis which is local to the patellar component. See Figure 1.
NOTE 1 the origin of the coordinate system relative to the femoral component is defined by:
- aligning the femoral flexion/extension and femoral superior/inferior axes as defined in items 3.6 and
3.7, respectively; and then
- aligning the imaginary plane which the femoral flexion/extension and femoral superior/inferior axes lie
on so that it passes through 0 ° of tibio-femoral flexion

femoral flexion/extension axis
nominal axis of rotation of the femoral component relative to the patellar component.
NOTE for condylar knees the femoral flexion/extension axis may be determined by:
- consider two imaginary points on each condyle of the femoral component at the points where they would be in
contact with an plane perpendicular to the tibial axis at 30 ° and 60 ° of tibio-femoral flexion; and then

- visualising four lines, one from each of the imaginary points, perpendicular to the imaginary plane at each of the
flexion angles. The intersection of the pair of lines defined for each condyle will define two discrete points in
space. The flexion/extension axis is the line connecting these two points. See Figure 1.

line of action perpendicular to the femoral flexion/extension axis, passing through the centre of the femoral
components intercondylar notch.

femoral anterior/posterior axis
line of action perpendicular to the femoral flexion/extension axis and the femoral superior/inferior axis.
3.5
patellar internal/external rotation axis
line of action passing through the centroid of area of the patellar backing and on the imaginary plane defined
by the backing of the patellar component.
NOTE rotational alignment of the patellar component to the patellar internal/external rotation axis should be
made following the manufacturer’s instructions for positioning of the component.
3.6
patellar floating axis
floating axis defined as the common perpendicular to the femoral flexion/extension axis and the patellar
internal/external rotation axis.
3.7
patellar compression force axis
line of action that passes through the centroid of area of the patellar component’s backing and is parallel to
the patellar abduction/adduction axis through which the patellar compression force is applied.
3.8
patello-femoral flexion/extension
angular displacement of the femoral component relative to the patellar component about the femoral
flexion/extension axis.
3.9
patello-femoral superior/inferior displacement
translational displacement of the patellar component relative to the femoral component along femoral
superior/inferior axis.
3.10
patello-femoral abduction/adduction
rotational displacement of the patellar component relative to the femoral component about the patellar
compression force axis. Commonly referred to as patellar rotation.
3.11
patello-femoral medial/lateral displacement
translational displacement of the patellar component relative to the femoral component along femoral
flexion/extension axis. Commonly referred to as patellar shift.
3.12
patello-femoral internal/external rotation
rotational displacement of the patellar component relative to the femoral component about the patellar
interior/exterior rotation axis. Commonly referred to as patellar tilt.

patellar compression force
compressive force applied between the patellar and femoral components of the knee-joint prosthesis along
the path of the patella compression force axis.

NOTE  the force is considered to be positive when it is compressive.
3.14
reference position
angular and translation alignment of the patellar component relative to the femoral component which give
static equilibrium of the patellar component when it is loaded against the femoral component by a negative
patellar compression force, with the most posterior region of the patellar component bearing surface resting
on the deepest point of the femoral components trochlear groove, the patellar superior/inferior axis is parallel to the
femoral superior/inferior axis and the patellar compression force passes through the femoral flexion/extension axis.
4 Principle
The patellar and femoral components of a test specimen are mounted in the test apparatus which applies a
specified time-varying force between the components, together with specified relative angular and
translational displacements, simulating normal human walking. The applied contact force/displacement
actions are patellar compression force, patello-femoral flexion/extension rotation, patello-femoral
superior/inferior displacement and patello-femoral abduction/adduction rotation. All the applied
force/displacement actions follow a specified cyclic variation with a fixed relationship between the phases of
the actions. Patello-femoral medial/lateral displacement and internal/external rotation are unconstrained
degrees of freedom.

les composants rotuliens et fémoraux. Ce système est composé : d’un système de coordonnées cartésiennes
fixé de manière rigide au composant fémoral, comprenant l’axe de flexion/extension fémorale, l’axe fémoral
supérieur/inférieur et l’axe fémoral antérieur/postérieur ; de l’axe de rotation interne/externe rotulienne qui est
fixé au composant rotulien ; d’un axe flottant et de l’axe de compression rotulienne qui est localisé sur le

4 Principe
Les composants rotuliens et fémoraux d’une éprouvette d’essai sont montés dans l’appareillage d’essai qui
applique une force spécifiée qui varie en fonction du temps entre les composants, ainsi que des
déplacements relatifs angulaire et de translation pour simuler une marche humaine normale. La force de
contact appliquée et les déplacements sont la force de compression rotulienne, la rotation de
flexion/extension fémoro-patellaire, le déplacement supérieur/inférieur fémoro-patellaire et la rotation
d’abduction/adduction fémoro-patellaire. La force appliquée et les déplacements suivent tous des variations
cycliques spécifiques avec une relation constante entre leurs phases. Le déplacement médial/latéral fémoro-
patellaire et la rotation interne/externe sont des degrés de liberté sans contrainte.
Les surfaces de contact des composants rotulien et fémoral sont immergées dans un liquide simulant le
liquide synovial humain. Si les polymères font l’objet d’une investigation, une éprouvette témoin est soumise
au milieu liquide et facultativement à la même force qui varie en fonction du temps, afin de déterminer le
fluage de l’éprouvette d’essai et/ou la variatio
