oSIST prEN IEC 60601-2-36:2026 is a SIST publication of the IEC particular standard for medical electrical equipment used in extracorporeally induced lithotripsy. It specifies particular requirements for basic safety and essential performance of therapy equipment that generates extracorporeally induced focused pressure pulses, so manufacturers, test teams, and clinical users can judge the device, its instructions, and its EMC behavior.
What does oSIST prEN IEC 60601-2-36:2026 specify?
oSIST prEN IEC 60601-2-36:2026 specifies the particular safety and performance requirements for therapy equipment for extracorporeally induced focused pressure pulses. It follows the IEC 60601 structure and adds requirements where the general standard needs to be adapted for lithotripsy equipment.
The standard applies only to components directly involved in the pressure pulse treatment, such as the pressure pulse generator and its interaction with patient support, imaging, monitoring, or treatment planning devices. Those other devices are covered by their own IEC standards. The document also gives priority to its particular requirements wherever they differ from the general standard.
| Annex | What it covers |
|---|---|
| AA | Informative guidance and rationale for selected clauses |
| BB | Informative coordinate system, FOCUS, TARGET LOCATION, and Figure BB.1 |
What are the key requirements of oSIST prEN IEC 60601-2-36:2026?
oSIST prEN IEC 60601-2-36:2026 focuses on safe delivery of pressure pulses, correct reporting of output, and clear user information. In practice, the document asks manufacturers to control the treatment beam, describe its characteristics, and verify that the equipment behaves safely under fault and EMC-related conditions.
Scope, related standards, and priority rules
Clause 201.1 limits the standard to therapy equipment for extracorporeally induced focused pressure pulses and excludes ultrasound physiotherapy equipment and ultrasound equipment for high intensity therapeutic ultrasound (HITU). It also states that IEC 60601-1-2 applies as modified by Clause 202, while several other collateral standards do not apply. For a buyer or QA team, this means the standard is narrow in scope and must be read together with the named companion standards.
Clause 201.1.4 explains how this particular standard works with the general standard: it can replace, amend, or add requirements, and its requirements take priority. That matters when checking conformity, because the particular clauses are the first place to look for lithotripsy-specific rules.
Essential performance and treatment control
Clause 201.4.3.101 requires the ME EQUIPMENT to be free from incorrect display of energy levels and free from unintended shock wave release. Compliance is checked using the manufacturer’s testing method and by conformity inspection to IEC 60601-1-2. In practice, the energy display, release logic, and immunity behavior all need to be designed and verified together.
Clause 201.12.4.6 requires PRESSURE PULSE release to stay under the OPERATOR’s control through deliberate action. Clause 201.12.4.3 requires protection against unintended output levels to remain safe in SINGLE FAULT CONDITION, and Clause 201.12.4.4 addresses interlocking when more than one device can control the pressure pulse function. These clauses matter because they prevent accidental firing or uncontrolled output during normal use and foreseeable faults.
Information for users and technical description
Clause 201.7.9.2 requires the instructions for use to warn about pressure pulses delivered to gas-containing organs, proper PRESSURE PULSE COUPLING, and attenuation through tissue with extra absorption in bone. It also ties other safety precautions to the RISK MANAGEMENT FILE. That means the user information must reflect the device’s real hazards and the manufacturer’s risk control decisions.
Clause 201.7.9.3 requires technical description data such as positional precision of the TARGET MARKER, the position and size of the FOCAL VOLUME, acoustic pressure values, ENERGY FLUX DENSITY, and ENERGY PER PULSE at minimum, typical, and maximum settings. The section also points to Annex BB for the coordinate system. In practice, this is the data set buyers and evaluators use to compare systems and understand where and how the treatment is delivered.
Electrical, mechanical, and EMC provisions
Clause 201.8.7.1 changes leakage-current testing so PATIENT LEAKAGE CURRENT is not measured during PRESSURE PULSE release. Clause 201.8.8.3 adjusts dielectric-strength testing of insulation used as a MEANS OF PROTECTION and a MEANS OF OPERATOR PROTECTION. This reflects the high-voltage nature of the equipment and tells test laboratories when the normal electrical test conditions need adaptation.
Clause 201.9.6.2.1 sets exposure-based limits for audible acoustic energy and asks manufacturers to consider noise protection when the A-weighted sound pressure level is high. Clause 201.9.8.4.1 adds conditions for ropes, chains, belts, and wire ropes used as anti-drop devices. For users, this means the standard covers both acoustic nuisance and the physical safety of moving or supported parts.
Clause 201.11.8 requires deliberate operator action before PRESSURE PULSE release can resume after power is restored. Clause 201.15.4.7.1 requires a specified minimum force for foot switches. These are practical usability and safety controls that reduce accidental activation.
Clause 201.12.1.101 to 201.12.1.103 require manufacturers to provide methods for checking target-marker alignment, detecting deviations, and performing constancy testing over the life of the equipment. Clause 202 keeps IEC 60601-1-2 applicable, except during the triggering and generation cycle of PRESSURE PULSE release. That is important because the device must be assessed for EMC like other medical electrical equipment, while recognizing the special high-energy pulse phase.
What terms does oSIST prEN IEC 60601-2-36:2026 define?
- ENERGY FLUX DENSITY - the derived pulse-intensity integral at the FOCUS, used to describe the local acoustic exposure of the pressure pulse field.
- ENERGY PER PULSE - the derived acoustic pulse energy over a chosen cross-sectional area, with the integration limits and area definition stated for interpretation.
- EXTRACORPOREALLY INDUCED LITHOTRIPSY - lithotripsy inside the patient using pressure pulses generated outside the patient.
- FOCAL VOLUME - the volume enclosed by the -6 dB or n MPa isobar of the maximum peak compressional acoustic pressure.
- LITHOTRIPSY EQUIPMENT - ME EQUIPMENT intended for lithotripsy treatment.
- LOCALIZATION DEVICE - a device used to determine the position of calculi in three-dimensional space.
- POSITIONING DEVICE - a device that brings the calculi into coincidence with the TARGET LOCATION.
- PRESSURE PULSE - the acoustic wave emitted by the ME EQUIPMENT.
Who uses oSIST prEN IEC 60601-2-36:2026?
oSIST prEN IEC 60601-2-36:2026 is used by manufacturers of lithotripsy systems and by the teams who write their instructions for use, technical descriptions, and risk management files. It is also relevant to test laboratories and conformity assessment teams that check electrical safety, EMC, acoustic output, target alignment, and release controls. In hospitals and service organizations, it helps clinical engineering and maintenance staff understand the safety features that affect setup and operation.
What changed in oSIST prEN IEC 60601-2-36:2026 from the previous edition?
oSIST prEN IEC 60601-2-36:2026 is the third edition and it cancels and replaces the second edition published in 2014. The foreword says this edition is a technical revision aligned structurally with IEC 60601-1:2005 and its amendments. The introduction also explains that the title was broadened from a description centered on ESWL to the more general physical mode of action, extracorporeally induced focused pressure pulses.
Which standards are used with oSIST prEN IEC 60601-2-36:2026?

124 INTRODUCTION
125 This particular standard concerns the BASIC SAFETY and ESSENTIAL PERFORMANCE of therapy
126 equipment for EXTRACORPOREALLY INDUCED FOCUSED PRESSURE PULSES. It amends and
127 supplements IEC 60601-1:2005, IEC 60601-1:2005/A1:2012 and IEC 60601-1:2005/A2:2020:
128 Medical electrical equipment – Part 1: General requirements for basic safety and essential
129 performance), hereinafter referred to as the general standard.
130 The aim of this third edition is to bring this particular standard up to date with reference to the
131 third edition of the general standard through reformatting and technical changes.
132 This particular standard applied to BASIC SAFETY and ESSENTIAL PERFORMANCE of equipment for
133 EXTRACORPOREALLY INDUCED LITHOTRIPSY as defined in 201.3.206 including equipment for other
134 medical applications of therapeutic EXTRACORPOREALLY INDUCED FOCUSED PRESSURE PULSES,
135 hereafter referred to as ME EQUIPMENT.
136 Only naming extracorporeal shock wave LITHOTRIPSY (ESWL) in the title was misleading
137 because that method does only describe the medical application for stone fragmentation
138 (LITHOTRIPSY) and does not cover other applications where for example tissue is stimulated by
139 extracorporeal shock wave therapy (ESWT).
140 In order to generalize the title and don´t restrict it to a medical application method the title
141 was changed as following.
142 “Particular requirements for the BASIC SAFETY and ESSENTIAL PERFORMANCE of therapy
143 equipment for EXTRACORPOREALLY INDUCED FOCUSED PRESSURE PULSES”.
144 ESWL and ESWT are both representing therapies where EXTRACORPOREALLY INDUCED FOCUSED
145 PRESSURE PULSES are used. The title change reflects the change from a possibly restricted
146 description of the medical application method to the universal description of the physical mode
147 of action.
148 The requirements of this particular standard take priority over those of the general standard.
149 A “General guidance and rationale” for the more important requirements of this particular
150 standard is included in Annex AA. It is considered that knowledge of the reasons for these
151 requirements will not only facilitate the proper application of the standard but will, in due course,
152 expedite any revision necessitated by changes in clinical practice or as a result of developments
153 in technology. However, Annex AA does not form part of the requirements of this standard.
