Non-invasive sphygmomanometers — Part 2: Clinical investigation of automated measurement type

ISO 81060-2:2013 specifies the requirements and methods for the clinical investigation of me equipment used for the intermittent non-invasive automated estimation of the arterial blood pressure by utilizing a cuff. ISO 81060-2:2013 is applicable to all sphygmomanometers that sense or display pulsations, flow or sounds for the estimation, display or recording of blood pressure. These sphygmomanometers need not have automatic cuff inflation. ISO 81060-2:2013 covers sphygmomanometers intended for use in all patient populations and all conditions of use. ISO 81060-2:2013 specifies additional disclosure requirements for the accompanying documents of sphygmomanometers that have undergone clinical investigation according to ISO 81060-2:2013.

Предисловие
Международная организация по стандартизации (ISO) является всемирной федерацией национальных
организаций по стандартизации (комитетов-членов ISO). Разработка международных стандартов
обычно осуществляется техническими комитетами ISO. Каждый комитет-член, заинтересованный в
деятельности, для которой был создан технический комитет, имеет право быть представленным в этом
комитете. Международные правительственные и неправительственные организации, имеющие связи с
ISO, также принимают участие в работах. Что касается стандартизации в области электротехники, то

данный стандарт были добавлены значения в кПа, эквивалентные значениям в мм рт. ст., включая критерий
2 требований 5.2.4.1.2. Оно также включает Техническую поправку ISO 81060-2:2009/Cor 1:2011.
ISO 81060-2 был подготовлен Техническим комитетом ISO/TC 121, Оборудование для анестезии и
искусственной вентиляции легких, Подкомитетом SC 3, Вентиляторы легких и сопутствующее
оборудование, совместно с техническим комитетом IEC/TC 62, Электрооборудование в медицинской
практике, Подкомитетом SC 62D, Электромедицинское оборудование в соответствии с 5 режимом
сотрудничества ISO/IEC.

Обращаем внимание комитетов-участников и национальных комитетов на факт того, что
производители оборудования и испытательные организации могут нуждаться в переходном периоде
после публикации новой, дополненной или пересмотренной, публикации ISO или IEC, в течение
которого они приведут продукты в соответствие с новыми требованиями и оснастятся для проведения
новых или пересмотренных испытаний. ISO/TC 121 и IEC/TC 62 рекомендуют, чтобы содержание

Введение
Определение КРОВЯНОГО ДАВЛЕНИЯ является важной процедурой, которая клинически используется для
оценки здоровья ПАЦИЕНТА.
Частое определение КРОВЯНОГО ДАВЛЕНИЯ является рутинной процедурой во время анестезии.
КРОВЯНОЕ ДАВЛЕНИЕ является вспомогательным параметром при титрации лекарств и инфузионной
терапии и может предупреждать об условиях, которые могут влиять на заболеваемость и смертность
ПАЦИЕНТОВ.

1 Область применения
Данная часть ISO 81060 определяет требования и методы КЛИНИЧЕСКИХ ИССЛЕДОВАНИЙ
СМЭ ОБОРУДОВАНИЯ, используемого для периодической неинвазивной автоматической оценки
артериального КРОВЯНОГО ДАВЛЕНИЯ, используя МАНЖЕТУ.
Данная часть ISO 81060 применима ко всем СФИГМОМАНОМЕТРАМ, которые чувствуют или отображают
пульсации, поток или звуки для оценки, отображения или записи КРОВЯНОГО ДАВЛЕНИЕ. Эти
СФИГМОМАНОМЕТРЫ не должны иметь автоматической накачки МАНЖЕТЫ.
Данная часть ISO 81060 покрывает СФИГМОМАНОМЕТРЫ, предназначенные для использования у всех
групп ПАЦИЕНТОВ (например, всех возрастов и весовых категорий), и при всех условиях использования
(например, амбулаторный контроль КРОВЯНОГО ДАВЛЕНИЯ, контроль КРОВЯНОГО ДАВЛЕНИЯ с нагрузкой и

ПРИМЕР Автоматические СФИГМОМАНОМЕТРЫ, определенные IEC 80601-2-30, подвергаются КЛИНИЧЕСКИМ
ИССЛЕДОВАНИЯМ согласно данной части ISO 81060.
Данная часть ISO 81060 определяет дополнительные требования к СОПРОВОДИТЕЛЬНОЙ ДОКУМЕНТАЦИИ
СФИГМОМАНОМЕТРОВ, которые подвергались КЛИНИЧЕСКИМ ИССЛЕДОВАНИЯМ в соответствии с данной
частью ISO 81060.

4.1 Методы валидации
СФИГМОМАНОМЕТРЫ, отличные от НЕАВТОМАТИЧЕСКИХ СФИГМОМАНОМЕТРОВ должны подвергаться
КЛИНИЧЕСКИМ ИССЛЕДОВАНИЯМ либо с использованием неинвазивного (аускультативного) ЭТАЛОННОГО
СФИГМОМАНОМЕТРА, либо используя ЭТАЛОННОЕ ИНВАЗИВНОЕ ОБОРУДОВАНИЕ ДЛЯ КОНТРОЛЯ КРОВЯНОГО
ДАВЛЕНИЯ в соответствии с данной частью ISO 81060 в каждом режиме работы.
ПРИМЕР 1 Режим взрослых и новорожденных.
ПРИМЕР 2 Режим с быстрой и медленной скоростью спуска МАНЖЕТЫ.
КЛИНИЧЕСКИЕ ИССЛЕДОВАНИЯ следует считать ТИПОВЫМ ИСПЫТАНИЕМ.
Считается, что соответствие требованиям данного подраздела достигнуто, если выполнены
критерии соответствующих инспекций и испытаний в данной части ISO 81060.

4.2 Приемлемая клиническая практика
Все КЛИНИЧЕСКИЕ ИССЛЕДОВАНИЯ должны соответствовать требованиям ISO 14155. КЛИНИЧЕСКИЕ
ИССЛЕДОВАНИЯ с ЭТАЛОННЫМ ИНВАЗИВНЫМ ОБОРУДОВАНИЕМ ДЛЯ КОНТРОЛЯ КРОВЯНОГО ДАВЛЕНИЯ не должны
проводиться на ПАЦИЕНТАХ или лицах только в целях исследования рабочих характеристик
СФИГМОМАНОМЕТРА.
ПРИМЕЧАНИЕ Некоторые компетентные органы имеют дополнительные требования.
Проверьте соответствие применением требований ISO 14155.
5 КЛИНИЧЕСКИЕ ИССЛЕДОВАНИЯ с аускультативным ЭТАЛОННЫМ СФИГМОМАНОМЕТРОМ
5.1 Требования к субъекту
5.1.1 * Количество
КЛИНИЧЕСКИЕ ИССЛЕДОВАНИЯ с аускультативным ЭТАЛОННЫМ СФИГМОМАНОМЕТРОМ должны состоять из,
как минимум, 85 субъектов. Если не определено иное, для каждого субъекта должно быть проведено,
по крайней мере, три валидных ОПРЕДЕЛЕНИЯ КРОВЯНОГО ДАВЛЕНИЯ. Должно быть, как минимум, 255
валидных парных определения КРОВЯНОГО ДАВЛЕНИЯ.
Проверьте соответствие инспекцией ОТЧЕТА О КЛИНИЧЕСКИХ ИССЛЕДОВАНИЯХ.
5.1.2 * Распределение по полам
По крайней мере, 30 % субъектов должно быть мужчинами и, по крайней мере, 30 % субъектов должно
быть женщинами.
Проверьте соответствие инспекцией ОТЧЕТА О КЛИНИЧЕСКИХ ИССЛЕДОВАНИЯХ.
5.1.3 * Распределение по возрастам
Для СФИГМОМАНОМЕТРОВ, предназначенных для использования у взрослых ПАЦИЕНТОВ и/или ПАЦИЕНТОВ
подросткового возраста, возраст субъектов, включенных в КЛИНИЧЕСКИЕ ИССЛЕДОВАНИЯ должен быть
больше 12 лет.
ПРИМЕЧАНИЕ 1 Всего минимум 85 субъектов.
Для СФИГМОМАНОМЕТРОВ, дополнительно предназначенных для использования у детей в КЛИНИЧЕСКИЕ
ИССЛЕДОВАНИЯ должны быть включены 35 субъектов-детей в возрасте между 3 годами и 12 годами.
ПРИМЕЧАНИЕ 2 Всего минимум 85 субъектов.
Если СФИГМОМАНОМЕТР имеет специальный режим для детей, в этом режиме дети считаются
специальной выборкой ПАЦИЕНТОВ (см. 5.1.6). В этих исследованиях дети освобождаются от
требований 5.1.5 к распределению КРОВЯНОГО ДАВЛЕНИЯ.
Детей возрастом менее 3 лет не следует включать в КЛИНИЧЕСКИЕ ИССЛЕДОВАНИЯ, использующим
аускультативное ОПРЕДЕЛЕНИЕ за счет снятия показаний с ЭТАЛОННОГО СФИГМОМАНОМЕТРА.
Проверьте соответствие инспекцией СОПРОВОДИТЕЛЬНОЙ ДОКУМЕНТАЦИИ и ОТЧЕТА О КЛИНИЧЕСКИХ
ИССЛЕДОВАНИЯХ.

5.1.4 * Распределение размеров конечностей
Для СФИГМОМАНОМЕТРОВ, предназначенных для использования с МАНЖЕТОЙ одного размера:
⎯ по крайней мере, 40 % субъектов должны иметь обхват руки, который лежит в пределах верхней
половины определенного диапазона использования МАНЖЕТЫ и, по крайней мере, 40 % должны
иметь обхват руки, который лежит в пределах нижней половины, и
⎯ по крайней мере, 20 % субъектов должны иметь обхват руки, который лежит в пределах верхней
четверти определенного диапазона использования МАНЖЕТЫ и, по крайней мере, 20 % должны
иметь обхват руки, который лежит в пределах нижней четверти.
Для СФИГМОМАНОМЕТРОВ, предназначенных для использования с несколькими размерами МАНЖЕТЫ,

каждый размер МАНЖЕТЫ должен испытываться на, по крайней мере, от всех субъектов, где

5.1.5 * Распределение КРОВЯНОГО ДАВЛЕНИЯ
По крайней мере, 5 % показаний ЭТАЛОННОГО КРОВЯНОГО ДАВЛЕНИЯ должны иметь СИСТОЛИЧЕСКОЕ
КРОВЯНОЕ ДАВЛЕНИЕ u 100 мм рт. ст. (13, 33 кПа).
По крайней мере, 5 % показаний ЭТАЛОННОГО КРОВЯНОГО ДАВЛЕНИЯ должны иметь СИСТОЛИЧЕСКОЕ
КРОВЯНОЕ ДАВЛЕНИЕ W 160 мм рт. ст. (21, 33 кПа).
По крайней мере, 20 % показаний ЭТАЛОННОГО КРОВЯНОГО ДАВЛЕНИЯ должны иметь СИСТОЛИЧЕСКОЕ
КРОВЯНОЕ ДАВЛЕНИЕ W 140 мм рт. ст. (18,66 кПа).
По крайней мере, 5 % показаний ЭТАЛОННОГО КРОВЯНОГО ДАВЛЕНИЯ должны иметь СИСТОЛИЧЕСКОЕ
КРОВЯНОЕ ДАВЛЕНИЕ u 60 мм рт. ст. (8 кПа).
По крайней мере, 5 % показаний ЭТАЛОННОГО КРОВЯНОГО ДАВЛЕНИЯ должны иметь СИСТОЛИЧЕСКОЕ

5.1.6 * Специальные выборки ПАЦИЕНТОВ
СФИГМОМАНОМЕТРЫ, предназначенные для использования в специальных выборках ПАЦИЕНТОВ, если
существуют ОБЪЕКТИВНЫЕ ДОКАЗАТЕЛЬСТВА, что точность СФИГМОМАНОМЕТРА может быть
проблематичной в этих выборках ПАЦИЕНТОВ, должны подвергаться КЛИНИЧЕСКИМ ИССЛЕДОВАНИЯМ в
этих выборках ПАЦИЕНТОВ. См. Также Раздел 0.
ПРИМЕЧАНИЕ В Разделе 7 определены примеры специальных популяций ПАЦИЕНТОВ со специальными
требованиями.
Если СФИГМОМАНОМЕТРЫ подвергаются КЛИНИЧЕСКИМ ИССЛЕДОВАНИЯМ в соответствии с требованиями
5.1.1 и 5.2, они должны подвергаться КЛИНИЧЕСКИМ ИССЛЕДОВАНИЯМ на, по крайней мере, 35
дополнительных субъектах специальной выборки. Если СФИГМОМАНОМЕТРЫ не подвергаются
предварительным КЛИНИЧЕСКИМ ИССЛЕДОВАНИЯМ в соответствии с требованиями 5.1.1 и 5.2,

КЛИНИЧЕСКИЕ ИССЛЕДОВАНИЯ в соответствии с требованиями 5.1.1 и 5.2 должны состоять только из
субъектов из специальной выборки ПАЦИЕНТОВ.
Специальная выборка ПАЦИЕНТОВ должна быть определена в ясных терминах и рассматривать
следующие параметры: пол (см. 5.1.2), возраст (см. 5.1.3), размер руки (см. 5.1.4) и КРОВЯНОЕ ДАВЛЕНИЕ
(см. 5.1.5). Сводка по данной информации должна быть приведена в инструкции по эксплуатации.
Проверьте соответствие инспекцией СОПРОВОДИТЕЛЬНОЙ ДОКУМЕНТАЦИИ и ОТЧЕТА О КЛИНИЧЕСКИХ
ИССЛЕДОВАНИЯХ.
5.2 Метод КЛИНИЧЕСКИХ ИССЛЕДОВАНИЙ с ЭТАЛОННЫМ СФИГМОМАНОМЕТРОМ
5.2.1 * Подготовка субъекта
Если в инструкции по эксплуатации ИСПЫТЫВАЕМОГО СФИГМОМАНОМЕТРА не указано иное, расположите
субъект так, чтобы субъекту:
⎯ было комфортно;
ПРИМЕР Комфортно усадите так, чтобы ноги не перекрещивались, а ступни были на полу.
⎯ имел спинку, подлокотники и поддержку предплечья;
⎯ середина МАНЖЕТЫ была на уровне правого предсердья сердца.
Рекомендуется, чтобы субъект максимально расслабился и не разговаривал во время всей процедуры.
До считывания первых показаний должно пройти 5 мин.

5.2.2 * Подготовка наблюдателя
Наблюдатели должны быть обучены использованию соответствующих методик для проведения
ОПРЕДЕЛЕНИЯ КРОВЯНОГО ДАВЛЕНИЯ покоя, используя приемлемые клинические протоколы измерения
КРОВЯНОГО ДАВЛЕНИЯ. См. Ссылки [8], [28], [29], [32] и [45], содержащие дополнительную информацию.
Наблюдатели должны иметь достаточный опыт проведения ОПРЕДЕЛЕНИЯ КРОВЯНОГО ДАВЛЕНИЯ.
Все наблюдатели, записывающие показания ЭТАЛОННОГО СФИГМОМАНОМЕТРА не должны быть видимы
другим наблюдателям. Показания ИСПЫТЫВАЕМОГО СФИГМОМАНОМЕТРА не должны быть видимы любому
из этих наблюдателей.
ПРИМЕР 1 Использование третьего наблюдателя для записи показаний ИСПЫТЫВАЕМОГО СФИГМОМАНОМЕТРА.
ПРИМЕР 2 Использование электронных средств записи показаний ИСПЫТЫВАЕМОГО СФИГМОМАНОМЕТРА.
Проинструктируйте наблюдателей, чтобы они определяли ДИАСТОЛИЧЕСКОЕ КРОВЯНОЕ ДАВЛЕНИЕ как
последний слышимый тон Короткова (пятая фаза или K5), за исключением случаев, когда тоны
Короткова остаются слышимыми при спущенной МАНЖЕТЕ, и детей возрастом между 3 годами и
12 годами, у которых используется четвертая фаза (K4). Если у детей не прослушивается К4, либо
используется К5, либо субъект исключается.
ПРИМЕЧАНИЕ Кроме детей, К4 должен также использоваться у субъектов, у которых большая разница между
приглушением и исчезанием тонов (при этом второе порой стремиться к нулю мм рт. ст.).
Проинструктируйте наблюдателей, чтобы они записывали, какой тон Короткова использовался для
ОПРЕДЕЛЕНИЯ ДИАСТОЛИЧЕСКОГО КРОВЯНОГО ДАВЛЕНИЯ.
Тоны Короткова, используемые для ОПРЕДЕЛЕНИЯ ДИАСТОЛИЧЕСКОГО КРОВЯНОГО ДАВЛЕНИЯ в КЛИНИЧЕСКИХ
ИССЛЕДОВАНИЯХ, должны быть указаны в инструкции по эксплуатации СФИГМОМАНОМЕТРА.

5.2.3 * ЭТАЛОННОЕ ОПРЕДЕЛЕНИЕ
Два наблюдателя должны проводить одновременное ОПРЕДЕЛЕНИЕ КРОВЯНОГО ДАВЛЕНИЯ на каждом
субъекте, используя двойной стетоскоп.
Если испытываемый СФИГМОМАНОМЕТР не предназначен для использования при существенно
нерегулярном ритме сердце и если любой из наблюдателей определяет существенно нерегулярный
ритм сердца, это ОПРЕДЕЛЕНИЕ должно быть исключено.
ПРИМЕРЫ Бигеминия, тригеминия, изолированный VPB, мерцательная аритмия.
ПРИМЕЧАНИЕ 1 Хотя КЛИНИЧЕСКОЕ ИССЛЕДОВАНИЕ КРОВЯНОГО ДАВЛЕНИЯ у ПАЦИЕНТОВ с мерцательной аритмией
клинически важно, в настоящее время отсутствует общепринятое руководство по определению КРОВЯНОГО
ДАВЛЕНИЯ у таких лиц.

5.2.4 Методы КЛИНИЧЕСКИХ ИССЛЕДОВАНИЙ
5.2.4.1 Метод одновременного ОПРЕДЕЛЕНИЯ на одной руке
5.2.4.1.1 * Процедура
Этот метод должен использоваться только с ИСПЫТЫВАЕМЫМ СФИГМОМАНОМЕТРОМ:
⎯ который имеет МАНЖЕТУ, соответствующую ISO 81060-1;
⎯ который разработан для использования на предплечье; и
⎯ у которого:
⎯ постоянная линейная скорость спуска от 2 мм рт. ст./с (0,27 кПа/с) до 3 мм рт. ст./с (0,40 кПа/с)
или
⎯ для испытываемого СФИГМОМАНОМЕТРА, который контролирует спуск как функцию пульса,
скорость спуска от 2 мм рт. ст./удар (0,27 кПа/удар) до 3 мм рт. ст./удар (0,40 кПа/удар).
Может использоваться любая рука.
ИСПЫТЫВАЕМЫЙ СФИГМОМАНОМЕТР не должен спускать до ОПРЕДЕЛЕНИЯ ЭТАЛОННОГО ДИАСТОЛИЧЕСКОГО
КРОВЯНОГО ДАВЛЕНИЯ. ИСПЫТЫВАЕМЫЙ СФИГМОМАНОМЕТР может быть изменен, чтобы соответствовать
данному критерию.
ПРИМЕЧАНИЕ Корректное одновременное определение на одной руке требует, чтобы испытываемый
СФИГМОМАНОМЕТР надувал МАНЖЕТУ до уровня, по крайней мере, на 20 мм рт. ст. (2,67 кПа) выше, чем реальное

Выполните следующее:
a) Проведите наблюдение, используя одновременное ОПРЕДЕЛЕНИЕ КРОВЯНОГО ДАВЛЕНИЯ субъекта,
используя ЭТАЛОННЫЙ СФИГМОМАНОМЕТР и ИСПЫТЫВАЕМЫЙ СФИГМОМАНОМЕТР, используя одну и ту
же МАНЖЕТУ и один и тот же цикл накачки/спуска (см. Рисунок 1). Эти данные не используются
для вычисления точности ИСПЫТЫВАЕМОГО СФИГМОМАНОМЕТРА.
b) Очистите память ИСПЫТЫВАЕМОГО СФИГМОМАНОМЕТРА от предыдущего ОПРЕДЕЛЕНИЯ и затем
выждите, по крайней мере, 60 с.
ПРИМЕРЫ Методы очистки памяти от предыдущего ОПРЕДЕЛЕНИЯ: выключение и включение питания,
удаление модуля КРОВЯНОГО ДАВЛЕНИЯ и повторная его установка или запуск команды перезагрузки.
c) Проведите наблюдение, используя одновременное ОПРЕДЕЛЕНИЕ КРОВЯНОГО ДАВЛЕНИЯ субъекта,
используя ЭТАЛОННЫЙ СФИГМОМАНОМЕТР и ИСПЫТЫВАЕМЫЙ СФИГМОМАНОМЕТР, используя одну и ту
же МАНЖЕТУ и один и тот же цикл накачки/спуска.
d) Выждите, по крайней мере, 60 с между ОПРЕДЕЛЕНИЯМИ.
e) Повторите с) и d) пока не будет проведено необходимое количество ОПРЕДЕЛЕНИЙ.
Все данные от субъекта должны быть исключены, если любые два ЭТАЛОННЫХ ОПРЕДЕЛЕНИЯ

эталонных ОПРЕДЕЛЕНИЯ ДИАСТОЛИЧЕСКОГО кровяного давление отличаются более чем на 8 мм рт. ст.
(1,07 кПа).

Foreword
ISO (the International Organization for Standardization) is a worldwide federation of national standards bodies
(ISO member bodies). The work of preparing International Standards is normally carried out through ISO
technical committees. Each member body interested in a subject for which a technical committee has been

Introduction
Determination of BLOOD PRESSURE is an important procedure that is clinically used to assess the status of a
PATIENT.
Frequent determination of BLOOD PRESSURE is routine during anaesthesia. BLOOD PRESSURE serves to aid in
drug titration and fluid management and to provide warning of conditions that could affect PATIENT morbidity
and mortality.

Clinical investigation of the automated measurement type
1 Scope
This part of ISO 81060 specifies the requirements and methods for the CLINICAL INVESTIGATION of
ME EQUIPMENT used for the intermittent non-invasive automated estimation of the arterial BLOOD PRESSURE by
utilizing a CUFF.
This part of ISO 81060 is applicable to all SPHYGMOMANOMETERS that sense or display pulsations, flow or
sounds for the estimation, display or recording of BLOOD PRESSURE. These SPHYGMOMANOMETERS need not
have automatic CUFF inflation.
This part of ISO 81060 covers SPHYGMOMANOMETERS intended for use in all PATIENT populations (e.g. all age
and weight ranges), and all conditions of use (e.g. ambulatory BLOOD PRESSURE monitoring, stress testing
BLOOD PRESSURE monitoring and BLOOD PRESSURE monitors for the HOME HEALTHCARE ENVIRONMENT for self-

4 General requirements for CLINICAL INVESTIGATIONS
4.1 CLINICAL INVESTIGATION methods
SPHYGMOMANOMETERS other than NON-AUTOMATED SPHYGMOMANOMETERS shall undergo CLINICAL INVESTIGATION
either by using a non-invasive (auscultatory) REFERENCE SPHYGMOMANOMETER or by using REFERENCE INVASIVE
BLOOD PRESSURE MONITORING EQUIPMENT according to this part of ISO 81060 in each mode of operation.
EXAMPLE 1 Adult and neonatal modes.
EXAMPLE 2 Slow and fast CUFF deflation rate modes.
A CLINICAL INVESTIGATION shall be considered a TYPE TEST.
Consider compliance with the requirements of this subclause to exist when the criteria of the relevant
inspections and tests in this part of ISO 81060 are met.
4.2 Good clinical practice
All CLINICAL INVESTIGATIONS shall comply with the requirements of ISO 14155. CLINICAL INVESTIGATION with
REFERENCE INVASIVE BLOOD PRESSURE MONITORING EQUIPMENT should not be used for PATIENTS or subjects
solely for the purpose of investigating SPHYGMOMANOMETER performance.
NOTE Some authorities having jurisdiction have additional requirements.
The requirements of this International Standard, which are more specific than the corresponding requirements
of ISO 14155, shall prevail.

5 CLINICAL INVESTIGATION with an auscultatory REFERENCE SPHYGMOMANOMETER
5.1 Subject requirements
5.1.1 * Number
An auscultatory REFERENCE SPHYGMOMANOMETER CLINICAL INVESTIGATION shall consist of a minimum of 85
subjects. If not otherwise specified, at least three valid BLOOD PRESSURE DETERMINATIONS shall be taken for
each subject. There shall be a minimum of 255 valid paired BLOOD PRESSURE DETERMINATIONS.
Check compliance by inspection of the CLINICAL INVESTIGATION REPORT.
5.1.2 * Gender distribution
At least 30 % of the subjects shall be male and at least 30 % of the subjects shall be female.
Check compliance by inspection of the CLINICAL INVESTIGATION REPORT.
5.1.3 * Age distribution
For a SPHYGMOMANOMETER intended for use on adults and/or adolescent PATIENTS, the age of every subject
included in the CLINICAL INVESTIGATION shall be greater than 12 years.
NOTE 1 Minimum total of 85 subjects.
For a SPHYGMOMANOMETER additionally intended for use in children, 35 child subjects aged between 3 years
and 12 years shall be included in the CLINICAL INVESTIGATION.
NOTE 2 Minimum total of 85 subjects.
If the SPHYGMOMANOMETER has a special mode for children, in that mode, children shall be considered a
special PATIENT population (see 5.1.6). In such a study, children are exempt from the BLOOD PRESSURE
distribution requirements of 5.1.5.
Children aged less than 3 years shall not be included in a CLINICAL INVESTIGATION utilizing auscultatory
DETERMINATIONS by observers with a REFERENCE SPHYGMOMANOMETER.
Check compliance by inspection of the ACCOMPANYING DOCUMENT and the CLINICAL INVESTIGATION REPORT.
5.1.4 * Limb size distribution
For a SPHYGMOMANOMETER intended for use with a single CUFF size:
 at least 40 % of the subjects shall have a limb circumference which lies within the upper half of the
specified range of use of the CUFF and at least 40 % shall have a limb circumference within the lower half;
and
 at least 20 % of the subjects shall have a limb circumference which lies within the upper quarter of the
specified range of use of the CUFF and at least 20 % shall have a limb circumference within the lower
quarter.
For a SPHYGMOMANOMETER intended for use with multiple CUFF sizes, each CUFF size shall be tested on at

least of the subjects, where n is the number of CUFF sizes.

5.1.5 * BLOOD PRESSURE distribution
At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a SYSTOLIC BLOOD PRESSURE ≤ 100 mmHg
(13,33 kPa).
At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a SYSTOLIC BLOOD PRESSURE ≥ 160 mmHg
(21,33 kPa).
At least 20 % of the REFERENCE BLOOD PRESSURE readings shall have a SYSTOLIC BLOOD PRESSURE
≥ 140 mmHg (18,66 kPa).
At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a DIASTOLIC BLOOD PRESSURE ≤ 60 mmHg
(8,0 kPa).
At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a DIASTOLIC BLOOD PRESSURE
≥ 100 mmHg (13,33 kPa).
At least 20 % of the REFERENCE BLOOD PRESSURE readings shall have a DIASTOLIC BLOOD PRESSURE
≥ 85 mmHg (11,33 kPa).
Check compliance by inspection of the CLINICAL INVESTIGATION REPORT.
5.1.6 * Special PATIENT populations
A SPHYGMOMANOMETER that is intended for use in special PATIENT populations where there is OBJECTIVE
EVIDENCE that the accuracy of the SPHYGMOMANOMETER might be problematic in those PATIENT populations,
shall undergo CLINICAL INVESTIGATION in those PATIENT populations.
NOTE Clause 7 has a specific example of a special PATIENT population with specific requirements.
If the SPHYGMOMANOMETER has undergone CLINICAL INVESTIGATION according to the requirements of 5.1.1
and 5.2, it shall then undergo CLINICAL INVESTIGATION in at least an additional 35 special population subjects. If
the SPHYGMOMANOMETER has not previously undergone CLINICAL INVESTIGATION according to the requirements
of 5.1.1 and 5.2, the CLINICAL INVESTIGATION in accordance with the requirements of 5.1.1 and 5.2 shall consist
only of subjects from the special PATIENT population.
The special PATIENT population shall be defined in clear terms and address the following attributes: gender
(see 5.1.2), age (see 5.1.3), limb size (see 5.1.4) and BLOOD PRESSURE (see 5.1.5). A summary of this
information shall be disclosed in the instructions for use.
Check compliance by inspection of the instructions for use and the CLINICAL INVESTIGATION REPORT.
5.2 CLINICAL INVESTIGATION method with a REFERENCE SPHYGMOMANOMETER
5.2.1 * Subject preparation
Unless otherwise indicated by the instructions for use of the SPHYGMOMANOMETER-UNDER-TEST, position the
subject such that the subject:
 is comfortable;
EXAMPLE Comfortably seated with legs uncrossed and feet flat on the floor.
 has the back, elbow and forearm supported;
 has the middle of the CUFF at the level of the right atrium of the heart.

Recommend that the subject be as relaxed as possible and that the subject avoid talking during the entire
procedure. Before the first reading is taken, 5 min should elapse.
NOTE Additional details can be found in Reference [32].
5.2.2 * Observer preparation
Observers should be trained in using a proper methodology for performing a resting BLOOD PRESSURE
DETERMINATION by utilizing an accepted clinical protocol for BLOOD PRESSURE measurement.
References [8], [28], [29], [32] and [45] contain additional information. Observers should have sufficient
practice in performing BLOOD PRESSURE DETERMINATIONS.
Each observer's recording of observations of the REFERENCE SPHYGMOMANOMETER shall not be visible to the
other observer. The readings of the SPHYGMOMANOMETER-UNDER-TEST shall not be visible to either of these
observers.
EXAMPLE 1 Utilizing a third observer for recording the readings of the SPHYGMOMANOMETER-UNDER-TEST.
EXAMPLE 2 Utilizing an electronic means for recording the readings of the SPHYGMOMANOMETER-UNDER-TEST.
Instruct the observers to determine the DIASTOLIC BLOOD PRESSURE as the last audible Korotkoff sound (fifth
phase or K5), except when Korotkoff sounds are still audible with the CUFF deflated or in children between 3
years and 12 years of age, where the fourth phase (K4) is used. If K4 is not audible in a child, either K5 is
used or the subject is excluded.
NOTE Other than for children, K4 should be reserved for subjects in whom there is a large discrepancy between muffling
and disappearance (with the latter at times approaching zero mmHg).
Instruct the observers to record which Korotkoff sound has been used for the DETERMINATION of DIASTOLIC
BLOOD PRESSURE.
The Korotkoff sound used for DETERMINATION of DIASTOLIC BLOOD PRESSURE in the CLINICAL INVESTIGATION shall
be disclosed in the instructions for use of a SPHYGMOMANOMETER.
EXAMPLE 3 K5 was used on 65 subjects and K4 was used on 20 subjects.
5.2.3 * REFERENCE DETERMINATION
Two observers shall make simultaneous BLOOD PRESSURE DETERMINATIONS on each subject using a double
stethoscope.
Unless the SPHYGMOMANOMETER-UNDER-TEST is intended for use during significantly irregular heart rhythm and
if either observer detects significantly irregular heart rhythm, that DETERMINATION shall be excluded.
EXAMPLES Bigeminy, trigeminy, isolated ventricular premature beat (VPB), atrial fibrillation.
NOTE 1 Although CLINICAL INVESTIGATION of BLOOD PRESSURE in PATIENTS with atrial fibrillation is clinically important,
there are currently no generally accepted guidelines for determining the BLOOD PRESSURE in such individuals.
Any pair of observers' DETERMINATIONS with a difference greater than 4 mmHg (0,53 kPa) shall be excluded.
The observers' individual values of each DETERMINATION shall be averaged according to Formula (1) to create
the REFERENCE BLOOD PRESSURE DETERMINATION.

The observer-to-observer differences shall be reviewed after completing a set of pairs of test-REFERENCE
DETERMINATIONS. If any DETERMINATIONS are excluded, additional pair(s) of DETERMINATIONS shall be taken to
ensure that the required number of valid test-REFERENCE pairs are available. A maximum of eight pairs of
DETERMINATIONS should be taken.
Use a REFERENCE SPHYGMOMANOMETER that complies with the requirements of ISO 81060-1, except that the
maximum permissible error shall be  1 mmHg (0,13 kPa). Reading of the values on the REFERENCE
SPHYGMOMANOMETER should be as accurate as possible. When reading the value on the REFERENCE
SPHYGMOMANOMETER, the observers should avoid parallax errors and rounding.
NOTE 2 Rounding has a negative effect on the results of the CLINICAL INVESTIGATION.
NOTE 3 For the purposes of this part of ISO 81060, the CUFF is considered part of the REFERENCE SPHYGMOMANOMETER.
A CUFF that does not comply with ISO 81060-1 cannot be used.
5.2.4 CLINICAL INVESTIGATION methods
5.2.4.1 Same arm simultaneous method
5.2.4.1.1 * Procedure
This method shall only be used with a SPHYGMOMANOMETER-UNDER-TEST:
 that has a CUFF compliant with ISO 81060-1;
 that is designed for use on the upper arm; and
 where
 the continuous linear deflation rate is between 2 mmHg/s (0,27 kPa/s) and 3 mmHg/s (0,40 kPa/s) or
 for a SPHYGMOMANOMETER-UNDER-TEST that controls the deflation as a function of the pulse rate, the
deflation rate is between 2 mmHg/pulse (0,27 kPa/pulse) and 3 mmHg/pulse (0,40 kPa/pulse).
Either arm may be utilized.
The SPHYGMOMANOMETER-UNDER-TEST shall not deflate prior to the detection of the REFERENCE DIASTOLIC
BLOOD PRESSURE. The SPHYGMOMANOMETER-UNDER-TEST may be modified to meet this criterion.
NOTE Valid same arm simultaneous DETERMINATIONS require the SPHYGMOMANOMETER-UNDER-TEST to inflate the CUFF
to at least 20 mmHg (2,67 kPa) higher than the actual SYSTOLIC BLOOD PRESSURE, as determined by the REFERENCE
SPHYGMOMANOMETER, and to at least 20 mmHg (2,67 kPa) below the actual DIASTOLIC BLOOD PRESSURE, as determined by
the REFERENCE SPHYGMOMANOMETER.
Perform the following:
a) Have the observers using the REFERENCE SPHYGMOMANOMETER and the SPHYGMOMANOMETER-UNDER-TEST
simultaneously determine the subject's BLOOD PRESSURE utilizing the same CUFF and inflation/deflation
cycle (see Figure 1). These data points are not used in the calculation of accuracy of the
SPHYGMOMANOMETER-UNDER-TEST.
b) Clear the SPHYGMOMANOMETER-UNDER-TEST memory of the previous DETERMINATION and then wait at least
60 s.

EXAMPLES Switching the power off and on, removing the BLOOD PRESSURE module and reinstalling it or issuing a
reset command are methods to clear the memory of the previous DETERMINATION.
c) Have the observers using the REFERENCE SPHYGMOMANOMETER and the SPHYGMOMANOMETER-UNDER-TEST
simultaneously determine the subject's BLOOD PRESSURE utilizing the same CUFF and inflation/deflation
cycle.
d) Wait at least 60 s between DETERMINATIONS.
e) Repeat c) and d) until the required number of valid DETERMINATIONS has been performed.
All data from a subject shall be excluded if any two REFERENCE SYSTOLIC BLOOD PRESSURE DETERMINATIONS
differ by more than 12 mmHg (1,60 kPa) or if any two REFERENCE DIASTOLIC BLOOD PRESSURE DETERMINATIONS
differ by more than 8 mmHg (1,07 kPa).

57 Frequent determination of BLOOD PRESSURE is routine during anaesthesia. BLOOD PRESSURE serves to aid in
58 drug titration and fluid management and to provide warning of conditions that could affect PATIENT morbidity

63 1 Scope
64 This part of ISO 81060 specifies the requirements and methods for the clinical investigation of ME EQUIPMENT
65 used for the intermittent non-invasive automated estimation of the arterial BLOOD PRESSURE by utilizing a CUFF.
66 This part of ISO 81060 is applicable to all SPHYGMOMANOMETERS that sense or display pulsations, flow or
67 sounds for the estimation, display or recording of BLOOD PRESSURE. These SPHYGMOMANOMETERS need not
68 have automatic CUFF inflation. This part of ISO 81060 covers SPHYGMOMANOMETERS intended for use in all
69 PATIENT populations (e.g. all age and weight ranges), and all conditions of use (e.g. ambulatory BLOOD
70 PRESSURE monitoring, stress testing BLOOD PRESSURE monitoring and BLOOD PRESSURE monitors for the HOME
71 HEALTHCARE ENVIRONMENT or self-measurement).
72 EXAMPLE AUTOMATED SPHYGMOMANOMETER as given in IEC 80601-2-30 clinically investigated by this part of ISO 81060.
73 This part of ISO 81060 specifies additional disclosure requirements for the ACCOMPANYING DOCUMENTS of

108 SPHYGMOMANOMETERS other than NON-AUTOMATED SPHYGMOMANOMETERS shall be clinically investigated either
109 by using a non-invasive (auscultatory) REFERENCE SPHYGMOMANOMETER or by using REFERENCE INVASIVE
110 BLOOD PRESSURE MONITORING EQUIPMENT according to this part of ISO 81060 in each mode of operation.
111 EXAMPLE 1 Adult and neonatal mode.
112 EXAMPLE 2 Slow and fast CUFF deflation rate mode.
113 A clinical investigation shall be considered a TYPE TEST.
114 Consider compliance with the requirements of this subclause to exist when the criteria of the relevant
115 inspections and tests in this part of ISO 81060 are met.
116 4.2 Good clinical practice
117 All clinical investigations shall comply with the requirements of ISO 14155:2011. Clinical investigation with
118 REFERENCE INVASIVE BLOOD PRESSURE MONITORING EQUIPMENT should not be used for PATIENTS or subjects
119 solely for the purpose of investigating SPHYGMOMANOMETER performance.
120 NOTE Some authorities with jurisdiction have additional requirements.
121 The following requirements that are more specific to a SPHYGMOMANOMETER prevail on the corresponding
122 requirements of ISO 14155:2011.

124 5 Clinical investigation with auscultatory REFERENCE SPHYGMOMANOMETER
125 5.1 Subject requirements
126 5.1.1 * Number
127 An auscultatory REFERENCE SPHYGMOMANOMETER clinical investigation shall consist of a minimum of 85
128 subjects. If not otherwise specified, at least three valid BLOOD PRESSURE DETERMINATIONS shall be taken for
129 each subject. There shall be a minimum of 255 valid paired BLOOD PRESSURE DETERMINATIONS.
130 Check compliance by inspection of the CLINICAL INVESTIGATION report.
131 5.1.2 * Gender distribution
132 At least 30 % of the subjects shall be male and at least 30 % of the subjects shall be female.
133 Check compliance by inspection of the CLINICAL INVESTIGATION REPORT.
134 5.1.3 * Age distribution
135 For a SPHYGMOMANOMETER intended for use on adults and/or adolescent PATIENTS, the age of every subject
136 included in the clinical investigation shall be greater than 12 years.
137 NOTE 1 Minimum total of 85 subjects.
138 For a SPHYGMOMANOMETER additionally intended for use in children, 35 child subjects aged between 3 years
139 and 12 years shall be included in the clinical investigation.
140 NOTE 2 Minimum total of 85 subjects.
141 If the SPHYGMOMANOMETER has a special mode for children, in that mode, children shall be considered a
142 special PATIENT population (see 5.1.6). In such a study, children are exempt from the BLOOD PRESSURE
143 distribution requirements of 5.1.5.
144 Children aged less than 3 years shall not be included in a clinical investigation utilizing auscultatory
145 DETERMINATIONS by observers with a REFERENCE SPHYGMOMANOMETER.
146 Check compliance by inspection of the ACCOMPANYING DOCUMENT and the CLINICAL INVESTIGATION REPORT.
147 5.1.4 * Limb size distribution
148 For a SPHYGMOMANOMETER intended for use with a single CUFF size:
149 ⎯ at least 40 % of the subjects shall have a limb circumference which lies within the upper half of the
150 specified range of use of the CUFF and at least 40 % shall have a limb circumference within the lower half;
151 and
152 ⎯ at least 20 % of the subjects shall have a limb circumference which lies within the upper quarter of the
153 specified range of use of the CUFF and at least 20 % shall have a limb circumference within the lower
154 quarter.
155 For a SPHYGMOMANOMETER intended for use with multiple CUFF sizes, each CUFF size shall be tested on at
156 least 1/(2 × n) of the subjects, where n is the number of CUFF sizes.

157 Check compliance by inspection of the ACCOMPANYING DOCUMENT and the CLINICAL INVESTIGATION REPORT.
158 5.1.5 * BLOOD PRESSURE distribution
159 At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a SYSTOLIC BLOOD PRESSURE ≤ 100 mmHg
160 (13,33 kPa).
161 At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a SYSTOLIC BLOOD PRESSURE ≥ 160 mmHg
162 (21,33 kPa).
163 At least 20 % of the REFERENCE BLOOD PRESSURE readings shall have a SYSTOLIC BLOOD PRESSURE ≥
164 140 mmHg (18,66 kPa).
165 At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a DIASTOLIC BLOOD PRESSURE ≤ 60 mmHg
166 (8,0 kPa).
167 At least 5 % of the REFERENCE BLOOD PRESSURE readings shall have a DIASTOLIC BLOOD PRESSURE ≥
168 100 mmHg (13,33 kPa).
169 At least 20 % of the REFERENCE BLOOD PRESSURE readings shall have a DIASTOLIC BLOOD PRESSURE ≥
170 85 mmHg (11,33 kPa).
171 Check compliance by inspection of the CLINICAL INVESTIGATION REPORT.
172 5.1.6 * Special PATIENT populations
173 A SPHYGMOMANOMETER that is intended for use in special PATIENT populations where there is OBJECTIVE
174 EVIDENCE that the accuracy of the SPHYGMOMANOMETER might be problematic in those PATIENT populations,
175 shall be clinically investigated in those PATIENT populations.
176 NOTE Clause 7 has a specific example of a special PATIENT population with specific requirements.
177 If the SPHYGMOMANOMETER has been clinically investigated according to the requirements of 5.1.1 and 5.2, it
178 shall then be clinically investigated in at least an additional 35 special population subjects. If the
179 SPHYGMOMANOMETER has not been previously clinically investigated according to the requirements of 5.1.1
180 and 5.2, the clinically investigation in accordance with the requirements of 5.1.1 and 5.2 shall only consist of
181 subjects from the special PATIENT population.
182 The special PATIENT population shall be defined in clear terms and address the following attributes: gender
183 (see 5.1.2), age (see 5.1.3), limb size (see 5.1.4) and BLOOD PRESSURE (see 5.1.5). A summary of this
184 information shall be disclosed in the instructions for use.
185 Check compliance by inspection of the instructions for use and the CLINICAL INVESTIGATION REPORT.
186 5.2 Clinical investigation method with REFERENCE SPHYGMOMANOMETER
187 5.2.1 * Subject preparation
188 Unless otherwise indicated by the instructions for use of the SPHYGMOMANOMETER-UNDER-TEST, position the
189 subject such that the subject:
190 ⎯ is comfortable;
191 EXAMPLE Comfortably seated with legs uncrossed and feet flat on the floor.
192 ⎯ has the back, elbow and forearm supported;
193 ⎯ has the middle of CUFF at the level of the right atrium of the heart.

194 Recommend that the subject be as relaxed as possible and that the subject avoid talking during the entire
195 procedure. Before the first reading is taken, 5 min should elapse.
196 NOTE Additional details can be found in Reference [32].
197 5.2.2 * Observer preparation
198 Observers should be trained in using a proper methodology for performing a resting BLOOD PRESSURE
199 DETERMINATION by utilizing an accepted clinical protocol for BLOOD PRESSURE measurement. References [8],
200 [28], [29], [32] and [45] contain additional information. Observers should have sufficient practice in performing
201 BLOOD PRESSURE DETERMINATIONS.
202 Each observer's recording of observations of the REFERENCE SPHYGMOMANOMETER shall not be visible to the
203 other observer. The readings of the SPHYGMOMANOMETER-UNDER-TEST shall not be visible to either of these
204 observers.
205 EXAMPLE 1 Utilizing a third observer for recording the readings of the SPHYGMOMANOMETER-UNDER-TEST.
206 EXAMPLE 2 Utilizing an electronic means for recording the readings of the SPHYGMOMANOMETER-UNDER-TEST.
207 Instruct the observers to determine DIASTOLIC BLOOD PRESSURE as the last audible Korotkoff sound (fifth phase
208 or K5), except when Korotkoff sounds are still audible with the CUFF deflated or in children between 3 years
209 and 12 years of age, where the fourth phase (K4) is used. If K4 is not audible in a child, either K5 is used or
210 the subject is excluded.
211 NOTE Other than for children, K4 should be reserved for subjects in whom there is a large discrepancy between muffling
212 and disappearance (with the latter at times approaching zero mmHg).
213 Instruct the observers to record which Korotkoff sound has been used for the DETERMINATION of DIASTOLIC
214 BLOOD PRESSURE.
215 The Korotkoff sound used for DETERMINATION of DIASTOLIC BLOOD PRESSURE in the clinical investigation shall be
216 disclosed in the instructions for use of a SPHYGMOMANOMETER.
217 EXAMPLE 3 K5 was used on 65 subjects and K4 was used on 20 subjects.
218 5.2.3 * REFERENCE DETERMINATION
219 Two observers shall make simultaneous BLOOD PRESSURE DETERMINATIONS on each subject using a double
220 stethoscope.
221 Unless the SPHYGMOMANOMETER-UNDER-TEST is intended for use during significantly irregular heart rhythm and
222 if either observer detects significantly irregular heart rhythm, that DETERMINATION shall be excluded.
223 EXAMPLES Bigeminy, trigeminy, isolated VPB, atrial fibrillation.
224 NOTE 1 Although clinical investigation of BLOOD PRESSURE in PATIENTS with atrial fibrillation is clinically important, there
225 are currently no generally accepted guidelines for determining the BLOOD PRESSURE in such individuals.
226 Any pair of observers' DETERMINATIONS with a difference greater than 4 mmHg (0,53 kPa) shall be excluded.
227 The observers' individual values of each DETERMINATION shall be averaged according to Equation (1) to create
228 the REFERENCE BLOOD PRESSURE DETERMINATION.

234 The observer-to-observer differences shall be reviewed after completing a set of pairs of test-REFERENCE

236 ensure that the required number of valid test-REFERENCE pairs are available. A maximum of eight pairs of
237 DETERMINATIONS should be taken.
238 Use a REFERENCE SPHYGMOMANOMETER that complies with the requirements of ISO 81060-1, except that the
239 maximum permissible error shall be ± 1 mmHg (0,13 kPa). Reading of the values on the REFERENCE
240 SPHYGMOMANOMETER should be as accurate as possible. When reading the value on the REFERENCE
241 SPHYGMOMANOMETER, the observers should avoid parallax errors. Rounding has a negative effect on the
242 results of the clinical investigation.
243 NOTE 2 For the purposes of this standard, the CUFF is considered part of the REFERENCE SPHYGMOMANOMETER. A CUFF
244 that does not comply with ISO 81060-1 cannot be used.
245 5.2.4 Clinical investigation methods
246 5.2.4.1 Same arm simultaneous method
247 5.2.4.1.1 * Procedure
248 This method shall only be used with a SPHYGMOMANOMETER-UNDER-TEST:
249 ⎯ that has a CUFF compliant with ISO 81060-1;
250 ⎯ that is designed for use on the upper arm; and
251 ⎯ where
252 ⎯ the continuous linear deflation rate is between 2 mmHg/s (0,27 kPa/s) and 3 mmHg/s (0,40 kPa/s) or
253 ⎯ for a SPHYGMOMANOMETER-UNDER-TEST that controls the deflation as a function of the pulse rate, the
254 deflation rate is between 2 mmHg/pulse (0,27 kPa/pulse) and 3 mmHg/pulse (0,40 kPa/pulse).
255 Either arm may be utilized.
256 The SPHYGMOMANOMETER-UNDER-TEST shall not deflate prior to the detection of the REFERENCE DIASTOLIC
257 BLOOD PRESSURE. The SPHYGMOMANOMETER-UNDER-TEST may be modified to meet this criterion.
258 NOTE Valid same arm simultaneous DETERMINATIONS require the SPHYGMOMANOMETER-UNDER-TEST to inflate the CUFF
259 to at least 20 mmHg (2,67 kPa) higher than the actual SYSTOLIC BLOOD PRESSURE, as determined by the REFERENCE
260 SPHYGMOMANOMETER, and to at least 20 mmHg (2,67 kPa) below the actual DIASTOLIC BLOOD PRESSURE, as determined by

Avant-propos
L'ISO (Organisation internationale de normalisation) est une fédération mondiale d'organismes nationaux de
normalisation (comités membres de l'ISO). L'élaboration des Normes internationales est en général confiée
aux comités techniques de l'ISO. Chaque comité membre intéressé par une étude a le droit de faire partie du
comité technique créé à cet effet. Les organisations internationales, gouvernementales et non

5.2.4.3.1 et 6.2.4 ont fait l'objet d'une révision technique. De nombreux éclaircissements ont été ajoutés et les
valeurs en kPa équivalentes aux valeurs en mmHg ont été incluses dans la norme, y compris les exigences
du critère 2 du 5.2.4.1.2. Elle incorpore également le Rectificatif technique ISO 81060-2:2009/Cor 1:2011.
L'ISO 81060-2 a été élaborée par le comité technique ISO/TC 121, Matériel d'anesthésie et de réanimation
respiratoire, sous-comité SC 3, Ventilateurs pulmonaires et équipements connexes, en collaboration avec le
comité technique CEI/TC 62, Équipements électriques dans la pratique médicale, sous-comité 62D, Appareils
électromédicaux, conformément au mode de collaboration 5 de l'ISO/CEI.
L’ISO 81060 comprend les parties suivantes, présentées sous le titre général Sphygmomanomètres non
invasifs:

L’attention des comités membres et des comités nationaux est attirée sur le fait que les fabricants
d'équipements et les organismes d'essai peuvent avoir besoin d'une période de transition après la publication
d'une norme ISO ou CEI nouvelle, amendée ou révisée, pour mettre leurs produits en conformité avec les
nouvelles exigences et s'équiper afin de réaliser les nouveaux essais ou les essais révisés. L'ISO/TC 121 et
le CEI/TC 62 recommandent que le contenu de la présente partie de l'ISO 81060 ne soit adopté en vue de sa
mise en œuvre obligatoire à l'échelle nationale que trois ans au plus tôt après sa date de publication pour les
équipements de conception nouvelle, et cinq ans au plus tôt après sa date de publication pour les

Introduction
La détermination de la PRESSION ARTERIELLE est une procédure importante utilisée en pratique clinique pour
évaluer l'état d'un PATIENT.
En cours d’anesthésie, il est systématiquement procédé à une surveillance fréquente de la PRESSION
ARTERIELLE. La PRESSION ARTERIELLE aide au dosage de l’anesthésique et permet une meilleure gestion des
fluides; elle permet également de prévenir des états qui pourraient affecter la morbidité et la mortalité du
PATIENT.

1 Domaine d'application
La présente partie de l’ISO 81060 précise les exigences et les méthodes d'INVESTIGATION CLINIQUE des
APPAREILS ELECTROMEDICAUX utilisés pour estimer ponctuellement, de manière non invasive et automatique, la
PRESSION ARTERIELLE au moyen d’un BRASSARD.
La présente partie de l’ISO 81060 est applicable à tous les SPHYGMOMANOMETRES qui captent ou affichent des
pulsations, des flux ou des sons pour l’estimation, l’affichage ou l’enregistrement de la PRESSION ARTERIELLE. Il
n’est pas nécessaire que ces SPHYGMOMANOMETRES aient un dispositif de gonflage automatique du BRASSARD.
La présente partie de l’ISO 81060 couvre les SPHYGMOMANOMETRES utilisables pour toutes les populations de
PATIENTS (par exemple: toutes les tranches d’âge et gammes de poids) et dans toutes les conditions d’emploi
(par exemple: contrôle ambulatoire de la PRESSION ARTERIELLE, contrôle de la PRESSION ARTERIELLE lors
d’épreuves d’effort, auto-contrôle de la PRESSION ARTERIELLE en ENVIRONNEMENT DE SOINS A DOMICILE et
contrôle de la pression artérielle dans un établissement de soins de santé).
EXEMPLE SPHYGMOMANOMETRE AUTOMATIQUE tel que défini dans la CEI 80601-2-30 faisant l'objet d'une INVESTIGATION
CLINIQUE conformément à la présente partie de l’ISO 81060.
La présente partie de l’ISO 81060 spécifie des exigences supplémentaires de divulgation d’informations pour
les DOCUMENTS D’ACCOMPAGNEMENT des SPHYGMOMANOMETRES ayant fait l'objet d'une INVESTIGATION CLINIQUE
conformément à la présente partie de l’ISO 81060.
La présente partie de l’ISO 81060 n’est pas applicable à l'INVESTIGATION CLINIQUE des SPHYGMOMANOMETRES
NON AUTOMATIQUES tels que définis dans l’ISO 81060-1 ou des APPAREILS DE SURVEILLANCE DE LA PRESSION
SANGUINE PRELEVEE DIRECTEMENT tels que définis dans la CEI 60601-2-34.

4.1 Méthodes d'INVESTIGATION CLINIQUE
Les SPHYGMOMANOMETRES autres que les SPHYGMOMANOMETRES NON AUTOMATIQUES doivent faire l'objet d'une
INVESTIGATION CLINIQUE, soit en utilisant un SPHYGMOMANOMETRE DE REFERENCE non invasif (auscultatoire), soit
au moyen d’un APPAREIL DE SURVEILLANCE DE LA PRESSION SANGUINE PRELEVEE DIRECTEMENT, conformément à
la présente partie de l’ISO 81060 pour chaque mode de fonctionnement.
EXEMPLE 1 Modes adulte et nouveau-né.
EXEMPLE 2 Modes de vitesse de dégonflage lente et rapide du BRASSARD.
Une INVESTIGATION CLINIQUE doit être considérée comme un ESSAI DE TYPE.
Les exigences du présent paragraphe sont considérées satisfaites lorsque les critères des examens et essais
applicables de la présente partie de l’ISO 81060 sont remplis.
4.2 Bonnes pratiques cliniques
Toutes les INVESTIGATIONS CLINIQUES doivent satisfaire aux exigences de l'ISO 14155. Il convient de ne pas
effectuer d'INVESTIGATION CLINIQUE au moyen d'APPAREILS DE SURVEILLANCE DE LA PRESSION SANGUINE PRELEVEE
DIRECTEMENT sur les PATIENTS ou des sujets dans le seul but d'étudier les performances de
SPHYGMOMANOMETRES.
NOTE Certaines autorités compétentes ont des exigences supplémentaires.
Les exigences de la présente Norme internationale, qui sont plus spécifiques que les exigences
correspondantes de l'ISO 14155, doivent prévaloir.
La conformité est vérifiée selon les exigences de l’ISO 14155.

5 INVESTIGATION CLINIQUE avec un SPHYGMOMANOMETRE DE REFERENCE auscultatoire
5.1 Exigences relatives aux sujets
5.1.1 * Nombre de sujets
L’INVESTIGATION CLINIQUE au moyen d’un SPHYGMOMANOMETRE DE REFERENCE auscultatoire doit comprendre au
minimum 85 sujets. Sauf spécification contraire, chaque sujet doit faire l’objet d’au moins trois
DETERMINATIONS valides de la PRESSION ARTERIELLE. Il doit y avoir au minimum 255 paires de DETERMINATIONS
valides de la PRESSION ARTERIELLE.
La conformité est vérifiée par examen du RAPPORT d'INVESTIGATION CLINIQUE.
5.1.2 * Distribution par sexe
Au moins 30 % des sujets doivent être de sexe masculin et 30 % de sexe féminin.
La conformité est vérifiée par examen du RAPPORT D'INVESTIGATION CLINIQUE.
5.1.3 * Distribution par âge
Pour un SPHYGMOMANOMETRE destiné à des PATIENTS adultes et/ou adolescents, chaque sujet inclus dans
l’INVESTIGATION CLINIQUE doit être âgé de plus de 12 ans.
NOTE 1 Nombre total minimal: 85 sujets.
Pour un SPHYGMOMANOMETRE qui est en outre destiné à des enfants, 35 sujets âgés de 3 ans à 12 ans
doivent être inclus dans l’INVESTIGATION CLINIQUE.
NOTE 2 Nombre total minimal: 85 sujets.
Si le SPHYGMOMANOMETRE comporte un mode spécial pour enfants, les enfants doivent être considérés
comme une population spéciale de PATIENTS lorsque l’appareil est utilisé dans ce mode (voir 5.1.6). Dans une
telle étude, la population d’enfants est exemptée des exigences de distribution de la PRESSION ARTERIELLE
décrites en 5.1.5.
Les enfants âgés de moins de 3 ans ne doivent pas être inclus dans une INVESTIGATION CLINIQUE utilisant des
DETERMINATIONS auscultatoires réalisées par des observateurs avec un SPHYGMOMANOMETRE DE REFERENCE.
La conformité est vérifiée par examen du DOCUMENT D'ACCOMPAGNEMENT et du RAPPORT D'INVESTIGATION
CLINIQUE.
5.1.4 * Distribution des tailles de bras
Lorsque le SPHYGMOMANOMETRE est destiné à être utilisé avec une seule taille de BRASSARD:
 au moins 40 % des sujets doivent avoir une circonférence du bras s’inscrivant dans la moitié supérieure
de la plage spécifiée d’utilisation du BRASSARD et au moins 40 % doivent avoir une circonférence du bras
dans la moitié inférieure de cette même plage; et
 au moins 20 % des sujets doivent avoir une circonférence du bras s’inscrivant dans le quart supérieur de
la plage spécifiée d’utilisation du BRASSARD et au moins 20 % doivent avoir une circonférence du bras
dans le quart inférieur de cette même plage.
Lorsque le SPHYGMOMANOMETRE est destiné à être utilisé avec plusieurs tailles de BRASSARD, chaque taille de

5.1.5 * Distribution de la PRESSION ARTERIELLE
Au moins 5 % des lectures de PRESSION ARTERIELLE DE REFERENCE doivent avoir une PRESSION ARTERIELLE
SYSTOLIQUE ≤ 100 mmHg (13,33 kPa).
Au moins 5 % des lectures de PRESSION ARTERIELLE DE REFERENCE doivent avoir une PRESSION ARTERIELLE
SYSTOLIQUE ≥ 160 mmHg (21,33 kPa).
Au moins 20 % des lectures de PRESSION ARTERIELLE DE REFERENCE doivent avoir une PRESSION ARTERIELLE
SYSTOLIQUE ≥ 140 mmHg (18,66 kPa).
Au moins 5 % des lectures de PRESSION ARTERIELLE DE REFERENCE doivent avoir une PRESSION ARTERIELLE
DIASTOLIQUE ≤ 60 mmHg (8,0 kPa).
Au moins 5 % des lectures de PRESSION ARTERIELLE DE REFERENCE doivent avoir une PRESSION ARTERIELLE
DIASTOLIQUE ≥ 100 mmHg (13,33 kPa).
Au moins 20 % des lectures de PRESSION ARTERIELLE DE REFERENCE doivent avoir une PRESSION ARTERIELLE
DIASTOLIQUE ≥ 85 mmHg (11,33 kPa).
La conformité est vérifiée par examen du RAPPORT D'INVESTIGATION CLINIQUE.
5.1.6 * Populations spéciales de PATIENTS
Lorsqu’il existe une PREUVE OBJECTIVE que l’exactitude d’un SPHYGMOMANOMETRE conçu pour des populations
spéciales de PATIENTS pourrait poser problème, le SPHYGMOMANOMETRE doit faire l’objet d’une INVESTIGATION
CLINIQUE sur ladite population de PATIENTS.
NOTE L'Article 7 contient un exemple spécifique d'une population spéciale de PATIENTS ayant des exigences
spécifiques.
Si le SPHYGMOMANOMETRE a fait l'objet d'une INVESTIGATION CLINIQUE conformément aux exigences du 5.1.1 et
du 5.2, il doit ensuite faire l'objet d'une INVESTIGATION CLINIQUE sur au moins 35 sujets supplémentaires de la
population spéciale. Si le SPHYGMOMANOMETRE n'a pas fait préalablement l'objet d'une INVESTIGATION CLINIQUE
conformément aux exigences du 5.1.1 et du 5.2, l'INVESTIGATION CLINIQUE conformément aux exigences
du 5.1.1 et du 5.2 doit être réalisée uniquement sur des sujets issus de la population spéciale de PATIENTS.
La population spéciale de PATIENTS doit être définie en termes clairs et faire état des attributs suivants: sexe
(voir 5.1.2), âge (voir 5.1.3), taille de bras (voir 5.1.4) et PRESSION ARTERIELLE (voir 5.1.5). Une synthèse de
ces informations doit être contenue dans les instructions d’utilisation.
La conformité est vérifiée par examen des instructions d'utilisation et du RAPPORT D'INVESTIGATION CLINIQUE.
5.2 Méthode d'INVESTIGATION CLINIQUE avec un SPHYGMOMANOMETRE DE REFERENCE
5.2.1 * Préparation des sujets
Sauf spécification contraire dans les instructions d’utilisation du SPHYGMOMANOMETRE EN ESSAI, positionner le
sujet de façon à ce:
 qu’il soit confortablement installé;
EXEMPLE Confortablement assis avec les jambes décroisées et les pieds à plat sur le sol.
 que son dos, ses coudes et ses avant-bras soient soutenus;

 que le milieu du BRASSARD soit situé au niveau de l’oreillette droite.
Il est recommandé que le sujet soit le plus détendu possible et qu’il évite de parler pendant toute la procédure.
Il convient d’attendre 5 min avant d’effectuer la première lecture.
NOTE Des détails supplémentaires sont donnés dans la référence [32].
5.2.2 * Préparation des observateurs
Il convient que les observateurs soient formés à l’application d’une méthodologie correcte de DETERMINATION
de la PRESSION ARTERIELLE au repos fondée sur un protocole clinique accepté de mesure de la PRESSION
ARTERIELLE. Les références [8], [28], [29], [32] et [45] contiennent des informations supplémentaires. Il
convient que les observateurs aient une pratique suffisante en matière de DETERMINATION de la PRESSION
ARTERIELLE.
L’enregistrement des mesures du SPHYGMOMANOMETRE DE REFERENCE effectuées par chaque observateur ne
doit pas être visible pour l’autre observateur. Les lectures du SPHYGMOMANOMETRE EN ESSAI ne doivent être
visibles pour aucun de ces observateurs.
EXEMPLE 1 Utilisation d’un troisième observateur pour enregistrer les lectures du SPHYGMOMANOMETRE EN ESSAI.
EXEMPLE 2 Utilisation d’un dispositif électronique pour enregistrer les lectures du SPHYGMOMANOMETRE EN ESSAI.
Instruire les observateurs pour que la détermination de la PRESSION ARTERIELLE DIASTOLIQUE soit effectuée au
dernier bruit audible de Korotkoff (cinquième phase ou K5), sauf lorsque les bruits de Korotkoff sont encore
audibles avec le BRASSARD dégonflé ou pour les enfants âgés de 3 ans à 12 ans, auquel cas la quatrième
phase (K4) est utilisée. Si K4 n'est pas audible chez un enfant, utiliser K5 ou exclure le sujet.
NOTE Outre les enfants, il convient de réserver K4 aux sujets présentant un important écart entre assourdissement et
disparition du bruit (quelquefois proche de zéro mmHg).
Instruire les observateurs pour qu’ils indiquent dans leurs enregistrements quel bruit de Korotkoff a été utilisé
pour la DETERMINATION de la PRESSION ARTERIELLE DIASTOLIQUE.
Le bruit de Korotkoff utilisé pour la DETERMINATION de la PRESSION ARTERIELLE DIASTOLIQUE dans le cadre de
l’INVESTIGATION CLINIQUE doit être mentionné dans les instructions d’utilisation d’un SPHYGMOMANOMETRE.
EXEMPLE 3 K5 a été utilisé sur 65 sujets et K4 a été utilisé sur 20 sujets.
5.2.3 * DETERMINATION DE REFERENCE
Deux observateurs doivent effectuer des DETERMINATIONS simultanées de la PRESSION ARTERIELLE de chaque
sujet au moyen d’un stéthoscope double.
Cette DETERMINATION doit être exclue à moins que le SPHYGMOMANOMETRE EN ESSAI ne soit conçu pour des
mesures impliquant des rythmes cardiaques substantiellement irréguliers et si les deux observateurs
détectent un rythme cardiaque substantiellement irrégulier.
EXEMPLES Bigéminisme, trigéminisme, extrasystole ventriculaire (ESV) isolée, fibrillation auriculaire.
NOTE 1 Bien que l’INVESTIGATION CLINIQUE de la PRESSION ARTERIELLE chez des PATIENTS présentant une fibrillation
auriculaire soit importante en pratique clinique, il n’existe pas actuellement de règles généralement admises pour la
détermination de la PRESSION ARTERIELLE de ces personnes.
Toute paire de DETERMINATIONS d’observateurs présentant une différence supérieure à 4 mmHg (0,53 kPa)
doit être exclue. La moyenne des valeurs individuelles de chaque DETERMINATION effectuée par les
observateurs doit être calculée selon la Formule (1) pour obtenir la DETERMINATION de la PRESSION ARTERIELLE
DE REFERENCE.

Les différences de résultats entre observateurs doivent être examinées une fois obtenu un ensemble de
paires de DETERMINATIONS de REFERENCE d’essai. Si d’éventuelles DETERMINATIONS sont exclues, une (des)
paire(s) de DETERMINATIONS supplémentaires doi(vent) être effectuée(s) pour disposer du nombre requis de
paires valides de REFERENCE d’essai. Il convient d’effectuer au maximum huit paires de DETERMINATIONS.
Utiliser un SPHYGMOMANOMETRE DE REFERENCE conforme aux exigences de l’ISO 81060-1, à l’exception du fait
que l’erreur maximale tolérée doit être de  1 mmHg (0,13 kPa). Il est souhaitable que la lecture des valeurs
du SPHYGMOMANOMETRE DE REFERENCE soit aussi exacte que possible. Il convient en l’occurrence que les
observateurs évitent les erreurs de parallaxe et d'arrondissage.
NOTE 2 L’arrondissage a un effet préjudiciable sur les résultats de l'INVESTIGATION CLINIQUE.
NOTE 3 Pour les besoins de la présente partie de l'ISO 81060, le BRASSARD est considéré comme faisant partie du
SPHYGMOMANOMETRE DE REFERENCE. Il n’est pas possible d’utiliser un BRASSARD non conforme à l’ISO 81060-1.
5.2.4 Méthodes d'INVESTIGATION CLINIQUE
5.2.4.1 Méthode de détermination simultanée sur un même bras
5.2.4.1.1 * Mode opératoire
Cette méthode ne doit être utilisée qu’avec un SPHYGMOMANOMETRE EN ESSAI:
 ayant un BRASSARD conforme à l'ISO 81060-1;
 conçu pour être utilisé sur la partie supérieure du bras; et
 dont
 la vitesse de dégonflage linéaire continue est de 2 mmHg/s (0,27 kPa/s) à 3 mmHg/s (0,40 kPa/s) ou
 si le dégonflage est contrôlé par un SPHYGMOMANOMETRE EN ESSAI en fonction de la fréquence du
pouls, la vitesse de dégonflage est comprise entre 2 mmHg/pulsation (0,27 kPa/pulsation) et
3 mmHg/pulsation (0,40 kPa/pulsation).
N'importe quel bras peut être utilisé.
Le SPHYGMOMANOMETRE EN ESSAI ne doit pas se dégonfler avant détection de la PRESSION ARTERIELLE
DIASTOLIQUE DE REFERENCE. Le SPHYGMOMANOMETRE EN ESSAI peut être modifié pour remplir ce critère.

NOTE Pour qu’elles soient valides, les DETERMINATIONS simultanées sur un même bras exigent que le
SPHYGMOMANOMETRE EN ESSAI puisse gonfler le BRASSARD à une pression supérieure d’au moins 20 mmHg (2,67 kPa) à la
PRESSION ARTERIELLE SYSTOLIQUE réelle, telle que déterminée par le SPHYGMOMANOMETRE DE REFERENCE, et inférieure d’au
moins 20 mmHg (2,67 kPa) à la PRESSION ARTERIELLE DIASTOLIQUE réelle, telle que déterminée par le SPHYGMOMANOMETRE
DE REFERENCE.
La procédure est la suivante:
a) Les observateurs utilisant le SPHYGMOMANOMETRE DE REFERENCE et le SPHYGMOMANOMETRE EN ESSAI
déterminent simultanément la PRESSION ARTERIELLE du sujet au moyen du même BRASSARD et selon le
même cycle de gonflage/dégonflage (voir Figure 1). Ces points de données ne sont pas utilisés pour le
calcul de l'exactitude du SPHYGMOMANOMETRE EN ESSAI.
b) Effacer de la mémoire du SPHYGMOMANOMETRE EN ESSAI la DETERMINATION précédente et attendre ensuite
au moins 60 s.
EXEMPLES La mise hors tension puis sous tension, le retrait du module de détermination de la PRESSION ARTERIELLE
et sa réinstallation ou une commande de réinitialisation sont des méthodes permettant d'effacer la DETERMINATION
précédente de la mémoire.
c) Les observateurs utilisant le SPHYGMOMANOMETRE DE REFERENCE et le SPHYGMOMANOMETRE EN ESSAI
déterminent simultanément la PRESSION ARTERIELLE du sujet au moyen du même BRASSARD et selon le
même cycle de gonflage/dégonflage.
d) Attendre au moins 60 s entre les DETERMINATIONS.
e) Répéter les points c) et d) jusqu'à ce que le nombre requis de DETERMINATIONS valides ait été effectué.
L’ensemble des données obtenues pour un sujet donné doit être exclu si deux DETERMINATIONS quelconques
de la PRESSION ARTERIELLE SYSTOLIQUE DE REFERENCE présentent une différence de plus de 12 mmHg
(1,60 kPa) ou si deux DETERMINATIONS quelconques de la PRESSION ARTERIELLE DIASTOLIQUE DE REFERENCE
présentent une différence de plus de 8 mmHg (1,07 kPa).

Le SPHYGMOMANOMETRE EN ESSAI doit satisfaire aux deux critères suivants:
