ISO 18310-1:2017 specifies suitable methods for the measurement of ambient dose equivalent rate at a distance from the patient treated with radioiodine to ablate the thyroid. For this purpose, direct measurement of the ambient dose equivalent rate due to the inpatients using an ionization chamber (or other suitable devices) may be employed. ISO 18310-1:2017 addresses the measurement methods, the calibration of ionization chamber and the uncertainty estimation for the measurement of the ambient dose equivalent rate of the patient treated with radioiodine to ablate the thyroid using the ionization chamber.

Introduction
ISO 18310 addresses measurement methods and procedures of ambient dose equivalent rate from
131
patients administered with I.
The incidence of thyroid cancer has increased in recent years. Thyroid cancer can be treated by
administering radioiodine, because radioiodine selectively accumulates in thyroid tissue to irradiate
and kill the cancerous cells. Thyroid cancers are small and not likely to develop into aggressive
malignancies. Earlier diagnosis and treatment can remove these cancers at a time when they are not
likely to have spread beyond the thyroid gland.
However, due to the radiation emitted from patients during treatment, the patients nearby or the
caregivers could also receive the dose. For this reason, a normative way to assess the dose to persons
close to the patient treated with radioiodine should be implemented. There are two common practices
for the treatment of thyroid cancer, one is a radioiodine administration without thyroid resection, and
the other is the administration after thyroid resection. In recent years, the radioiodine administration
after surgery has become more common.

The most commonly used radionuclides for the treatment is I. I mainly emits 364 keV of photon
energy with a few other photons and its radiological half-life is 8,02 d. The administered iodine is
absorbed in the digestive system, concentrated in the thyroid gland through blood circulation and after
a few hours, excreted into the bladder, and released through urine and faeces. For the patient who had
the thyroid removed, the retention time in the body is shorter than that for a patient who has not had
thyroid removal.
This document deals with the determination of ambient dose equivalent rate at a distance from the
patient treated with radioiodine therapy procedure. It is based on the estimation of the dose rate using
ionization chamber base dosimetry.
For the purpose of the ISO 18310 series, this document is focused on the determination of the ambient
dose equivalent rate from the patient. The uncertainty of the ambient dose equivalent is also provided.

1 Scope

4 Measurement of ambient dose equivalent
4.1 General
Measurement of ambient dose equivalent rate from a patient treated with radioiodine administration is
done using an ionization chamber as detailed below.
4.2 Calibration of the ionization chamber in the reference radiation
The calibration procedure of the ionization chamber with respect to ambient dose equivalent by the
national standard laboratory or the accredited laboratory is as follows.
The ionization chamber is calibrated using either the substitution method or the reference radiation
method. In the substitution method, the measurement using the reference ion chamber and the one
to be calibrated is performed at the same position with the exchange of the chambers. The calibration
coefficient is then determined from the ratio of two measured values. On the other hand, the ion
chamber calibration coefficient can be obtained by positioning it in the reference radiation field in
which the dose rate of each position was already determined by the calibration of the radiation field.
a) The user should be trained and competent in the facility’s calibration procedure prior to calibrating
an ion chamber.
b) Make sure that the ionization chamber to be calibrated is in proper working condition.

c) Confirm that the centre point of the chamber is on the central axis of the beam. The central axis of
the chamber stem is perpendicular to the direction of the beam and the marker or inscription on
the neck of the chamber faces the radiation source.
d) Determine that the beam size is greater than the size of the sensitive volume of the chamber by a
factor of 1,5 to 2.
e) Place the chamber at the calibration point and stabilize it after applying an appropriate voltage
through the measuring assembly.
f) Measure the leakage current or charge of the chamber at least five times before irradiation for
calibration purpose.
g) Establish and maintain environmental conditions for the calibration at 23 °C ± 2 °C, and at 50 % ±
20 % relative humidity during the measurement.
h) Upon irradiation from the standard irradiation system, measure the current or charge of the
chamber more than five times.
i) If the difference is more than ±0,3 % between the initial and final current measurements or more
than ±0,2 % in the atmospheric correction factor during the calibration, repeat steps f), g) and h).
j) Configuration of the irradiation system and the ionization chamber shall be arranged in accordance
with the condition in Figure 1.

4.3 Measurement of ambient dose equivalent

Measurement of the ambient dose equivalents for I using the ionization chamber during the clinical
experiments is as follows.
a) During hospitalization, at certain times after the radioiodine administration, the patient shall
be instructed to participate in the measurement of the current using the ionization chamber by
positioning the chamber 10 cm away from the neck of the patient and 1 m away from the patient.

b) Ambient dose equivalent due to I can be determined by multiplying the ambient dose equivalent

for the reference radiation by the ratio of the conversion coefficients between I and the reference

radiation. The conversion coefficient for I can be deduced as follows.

4.4 Examination of uncertainty elements
Examine the uncertainty elements involved in the measurement and estimate the total combined
uncertainty of the ambient dose equivalent from the mathematical model of the uncertainty calculation.
The uncertainty of the calibration coefficient of ionization chamber is provided from the calibration
certificate and it is a systematic uncertainty (type B). This uncertainty comprises the uncertainty
components of the reference calibration conditions such as the current measurement, reference air
kerma rate, positioning of the ionization chamber, the correction to the standard ambient conditions,
radiation field non-uniformity and conversion coefficients from air kerma to dose equivalent in the

reference radiation ( Cs or Co gamma-rays). Other uncertainty elements are the uncertainty
of current measurement (statistical uncertainty, type A), the uncertainty of correction of ambient
131
conditions (type B), the uncertainty of positioning the ionization chamber in I radiation (type B) and

the uncertainty of the ratio of the conversion coefficients between I and the reference radiations
(type B).
5 Mathematical model for a calibration of ionization chamber
5.1 General
The mathematical model for the calibration of ionization chambers with respect to ambient dose

6 Quality control
The ion chamber shall be calibrated to air kerma periodically by either a national laboratory or an
accredited laboratory. As per Bureau International des Poids et Mesures (BIPM) report, stability of the
calibration coefficient of the chamber shall be maintained within 1,5 %. The ionization chamber + the
cable + the electrometer shall be calibrated as a unit. The thermometer, barometer and hygrometer
shall also be calibrated periodically by the relevant accredited laboratory.
7 Uncertainty budget and estimation of uncertainty
7.1 Relative combined standard uncertainty
According to the uncertainty propagation law, relative combined standard uncertainty is given as a
square root of sum of the square of every uncertainty factor under consideration. Since the uncertainty
element for different calibration set ups may vary from one another and also consists of the combination
of multiplication and division of several quantities, the sensitivity of each component should be included
in the estimation of the combined uncertainty.
Sensitivity coefficient: all 1,0 because the quantities shown in the formula are independent of each other.
The uncertainty equation for the calibration coefficient of the ionization chamber calibrated in the
reference radiation is as given in Formula (4):

Effective degree of freedom should be determined to obtain the expanded standard uncertainty, which
can be done using the Welch–Salterthwaite formula given in Formula (6) according to the method
recommended in Reference [6].

Introduction
L’ISO 18310 décrit des méthodes et des modes opératoires de mesurage du débit d’équivalent de dose
ambiant dû à des patients ayant bénéficiés d’une administration d’iode 131.
De plus en plus de cancers de la thyroïde ont été diagnostiqués ces dernières années. Ces cancers
peuvent être traités par administration d’iode radioactif, car ce radionucléide s’accumule de manière
sélective dans les cellules thyroïdiennes, les irradie et tue les cellules cancéreuses. Les cancers de la
thyroïde sont peu volumineux et généralement peu agressifs. Le diagnostic et le traitement précoces
permettent d’éliminer ces cancers alors qu’ils n’ont pas encore eu le temps de s’étendre au-delà de la
glande thyroïde.
Cependant, en raison des rayonnements émis par les patients pendant le traitement, les patients à
proximité ou le personnel soignant peuvent également être exposés. Pour cette raison, il convient de
mettre en œuvre une méthode normative d’évaluation de la dose aux personnes à proximité du patient.
Il existe deux méthodes courantes pour traiter le cancer de la thyroïde: l’une consiste à administrer
de l’iode radioactif sans résection de la thyroïde, l’autre consiste à l’administrer après résection de la
thyroïde. Ces dernières années, l’administration d’iode radioactif après intervention chirurgicale s’est
répandue.
Le radionucléide le plus utilisé pour le traitement est l’iode 131. Il émet principalement de l’énergie
photonique de 364 keV, ainsi que d’autres émissions photoniques, et sa demi-vie radiologique est de
8,02 jours. L’iode administré est absorbé dans le système digestif, puis il se concentre dans la glande
thyroïde par l’intermédiaire de la circulation sanguine et après quelques heures, excrété dans la
vessie et éliminé dans les urines et les matières fécales. Pour le patient ayant bénéficié d’une ablation
de la thyroïde, le temps de rétention dans le corps est plus court que pour le patient n’ayant pas subi
d’ablation de la thyroïde.
Le présent document concerne la détermination du débit d’équivalent de dose ambiant à une certaine
distance du patient ayant bénéficié d’un traitement par iode radioactif. Il repose sur l’estimation du
débit de dose à l’aide de la dosimétrie basée sur une chambre d’ionisation.
Pour les besoins de la série ISO 18310, le présent document traite de la détermination du débit
d’équivalent de dose ambiant du patient. L’incertitude de l’équivalent de dose ambiant est également
abordée.

1 Domaine d’application
Le présent document spécifie des méthodes appropriées pour le mesurage du débit d’équivalent de dose
ambiant à une certaine distance du patient traité par iode radioactif pour l’ablation de la thyroïde. Pour
ce faire, le mesurage direct du débit d’équivalent de dose ambiant en raison des patients hospitalisés
peut être utilisé avec une chambre d’ionisation (ou par un autre dispositif approprié).
Le présent document concerne les méthodes de mesure, l’étalonnage de la chambre d’ionisation et
l’estimation de l’incertitude de mesure du débit d’équivalent de dose ambiant du patient traité par iode
radioactif pour l’ablation de la thyroïde à l’aide d’une chambre d’ionisation.

4 Mesurage de l’équivalent de dose ambiant
4.1 Généralités
Le mesurage du débit d’équivalent de dose ambiant d’un patient traité par administration d’iode
radioactif est effectué à l’aide d’une chambre d’ionisation, comme suit.
4.2 Étalonnage de la chambre d’ionisation avec le faisceau de référence
Le mode opératoire d’étalonnage, par le laboratoire national de métrologie ou par un laboratoire
accrédité, de la chambre d’ionisation à partir du mesurage de l’équivalent de dose ambiant, est le
suivant.
La chambre d’ionisation est étalonnée en utilisant soit la méthode de substitution, soit la méthode
d’irradiation de référence. Dans la méthode de substitution, le mesurage à l’aide de la chambre
d’ionisation de référence et celle à étalonner est effectué avec la même position en échangeant les
chambres. Le coefficient d’étalonnage est ensuite déterminé à partir du rapport des deux valeurs
mesurées. Par ailleurs, le coefficient d’étalonnage de la chambre d’ionisation peut être obtenu en
positionnant cette dernière dans le champ du faisceau de référence dans lequel le débit de dose à chaque
position a déjà été déterminé par l’étalonnage du champ du faisceau.
a) Il convient que l’utilisateur soit formé et connaisse parfaitement le mode opératoire d’étalonnage
du dispositif avant d’étalonner une chambre d’ionisation.
b) S’assurer que la chambre d’ionisation à étalonner est en état de marche.

c) Confirmer que le point central de la chambre se trouve sur l’axe central du faisceau, que l’axe
central de la tige de la chambre est perpendiculaire à la direction du faisceau et que le marquage ou
l’inscription sur le col de la chambre est en face de la source de rayonnement.
d) Déterminer que la taille du faisceau est supérieure à la taille du volume utile de la chambre d’un
facteur de 1,5 à 2.
e) Placer la chambre au niveau du point d’étalonnage et la stabiliser après avoir appliqué une tension
appropriée de l’unité de mesure.
f) Mesurer le courant ou la charge de fuite de la chambre au moins cinq fois avant irradiation à visée
d’étalonnage.
g) Établir et maintenir les conditions environnementales de l’étalonnage à 23 °C ± 2 °C et à 50 % ± 20 %
d’humidité relative pendant le mesurage.
h) Lors de l’irradiation par le système d’irradiation standard, mesurer le courant ou la charge de la
chambre plus de cinq fois.
i) En cas de variation supérieure à ± 0,3 % entre le premier et le dernier mesurage du courant, ou en
cas de variation supérieure à ± 0,2 % du facteur de correction atmosphérique pendant l’étalonnage,
répéter les étapes f), g) et h).
j) La configuration du système d’irradiation et de la chambre d’ionisation doit être conforme à la
Figure 1.

4.3 Mesurage de l’équivalent de dose ambiant

Le mesurage de l’équivalent de dose ambiant pour l’ I à l’aide d’une chambre d’ionisation pendant les
expériences cliniques est effectué de la façon suivante.
a) Pendant l’hospitalisation, à certains moments après administration de l’iode radioactif, le patient
est invité à participer au mesurage du courant électrique créé dans la chambre d’ionisation, suite à
son positionnement à 10 cm de son cou et à 1 m de lui.

b) L’équivalent de dose ambiant pour l’ I peut être déterminé en multipliant l’équivalent de dose

ambiant pour le faisceau de référence par le rapport des coefficients de conversion entre l I et

4.4 Examen des éléments d’incertitude
Examiner les éléments d’incertitude impliqués dans le mesurage et estimer l’incertitude composée
totale de l’équivalent de dose ambiant d’après le modèle mathématique du calcul de l’incertitude.
L’incertitude du coefficient d’étalonnage de la chambre d’ionisation est extraite du certificat d’étalonnage
et représente une incertitude systématique (type B). Cette incertitude comprend les composantes
d’incertitude des conditions d’étalonnage de référence telles que le mesurage du courant, le débit du
kerma de référence dans l’air, le positionnement de la chambre d’ionisation, la correction en fonction des
conditions environnementales standards, la non-uniformité du champ du faisceau et les coefficients de
conversion entre le kerma dans l’air et l’équivalent de dose avec le faisceau de référence (rayonnement

gamma du Cs ou du Co). Les autres éléments d’incertitude sont l’incertitude de mesure du courant
(incertitude statistique, type A), l’incertitude de correction des conditions environnementales (type B),

l’incertitude de positionnement de la chambre d’ionisation dans le rayonnement I (type B) et

l’incertitude du rapport des coefficients de conversion entre l’ I et le faisceau de référence (type B).
5 Modèle mathématique pour l’étalonnage de la chambre d’ionisation
5.1 Généralités
Le modèle mathématique pour l’étalonnage de la chambre d’ionisation sous la forme d’équivalent de

6 Contrôle qualité
La chambre d’ionisation doit être périodiquement étalonnée en termes de kerma dans l’air par un
laboratoire national ou par un laboratoire accrédité. Conformément au rapport du Bureau International
des Poids et Mesures (BIPM), la stabilité du coefficient d’étalonnage de la chambre doit être maintenue
à 1,5 % près. La chambre d’ionisation, le câble et l’électromètre doivent être étalonnés ensemble. Le

thermomètre, le baromètre et l’hygromètre doivent également être étalonnés périodiquement par un
laboratoire accrédité approprié.
7 Bilan des incertitudes et estimation de l’incertitude
7.1 Incertitude-type composée relative
D’après la loi de propagation des incertitudes, l’incertitude-type composée relative est donnée sous la
forme d’une racine carrée de la somme de chaque facteur d’incertitude en question. Étant donné que
les éléments d’incertitude de différentes configurations d’étalonnage peuvent varier et comprennent
également la multiplication et la division de plusieurs grandeurs, il convient d’inclure la sensibilité de
chaque composante dans l’estimation de l’incertitude composée.
Coefficient de sensibilité: 1,0 car les grandeurs indiquées dans la formule sont indépendantes les unes
des autres.
