ISO/IEEE FDIS 11073-10441
(Main)Health informatics — Device interoperability — Part 10441: Device specialization — Cardiovascular fitness and activity monitor
- Abstract
Within the context of the ISO/IEEE 11073 family of standards for device communication, a normative definition of communication between personal telehealth cardiovascular fitness and activity monitor devices and managers (e.g., cell phones, personal computers, personal health appliances, and set top boxes) is established in this standard in a manner that enables plug-and-play interoperability. Appropriate portions of existing standards are leveraged including ISO/IEEE 11073 terminology, information models, application profile standards, and transport standards. The use of specific term codes, formats, and behaviors in telehealth environments restricting optionality in base frameworks in favor of interoperability is specified. A common core of communication functionality for personal telehealth cardiovascular fitness and activity monitor devices is defined in this standard.

ISO/IEEE FDIS 11073-10441 - Health informatics — Device interoperability — Part 10441: Device specialization — Cardiovascular fitness and activity monitor
ISO/IEEE FDIS 11073-10441 is an ISO/IEEE device interoperability standard for personal telehealth cardiovascular fitness and activity monitor devices. It specifies how those devices communicate with managers such as cell phones, personal computers, personal health appliances, and set top boxes so that implementations can exchange data with a common core of object, service, and nomenclature definitions.
What does ISO/IEEE FDIS 11073-10441 specify?
ISO/IEEE FDIS 11073-10441 specifies the communication specialization for cardiovascular fitness and activity monitor devices within the ISO/IEEE 11073 family. It focuses on plug-and-play interoperability by fixing the term codes, formats, behaviors, objects, and communication choices that a manager and an agent use in telehealth environments.
The document is organized around the usual PHD building blocks: overview, normative references, definitions, the ISO/IEEE 11073 personal health device context, device concepts and modalities, the domain information model, the service model, the communication model, test associations, conformance, and two device profiles for step counter and activity monitor. Its annexes provide supporting ASN.1 definitions, nomenclature codes, bibliography material, and revision history.
| Annex | What it covers |
|---|---|
| Annex A | Informative bibliography |
| Annex B | Normative ASN.1 definitions added by this standard |
| Annex C | Normative nomenclature allocation, systematic names, deprecated and revised codes, discriminators, and alphabetical code listings |
| Annex D | Informative revision history |
The scope is narrow in a useful way: the standard defines the device specialization and the communication rules for this device type, while other safety, reliability, and risk management requirements are handled by other standards where applicable.
What are the key requirements of ISO/IEEE FDIS 11073-10441?
Communication is based on ISO/IEEE 11073-20601
ISO/IEEE FDIS 11073-10441 is built on ISO/IEEE 11073-20601 and uses its application profile, object model, and medical device encoding rules. Clause 4 states that the standard defines a subset of 20601 objects and functionality, then adds the definitions needed for the cardiovascular fitness and activity monitor specialization.
In practice, an implementation is not free to invent its own data model or transport behavior. It has to follow the shared 20601 framework and then apply the extra restrictions and specializations defined here so that managers can interpret the device data consistently.
The standard focuses on sessions, subsessions, and reporting style
Clause 5 defines how the device reports activity in a session and, when needed, in subsessions. A session is the envelope for one episode of exercise or activity, while a subsession is a portion of that episode, such as a lap, phase, or separate activity within the session.
This matters because the same device may report summary data at the end of an episode, periodic observations during the episode, or higher-rate raw samples during continuous monitoring. Clause 5 also distinguishes between numeric values and real-time sample array (RT-SA) data, which is used for high-rate waveform-like reporting such as 3D acceleration.
Measurements include fitness, activity, target, and sleep data
Clause 5 lists the measurement concepts used by this specialization, including 3D acceleration, 3D angular acceleration, activity intensity, altitude, cadence, distance, energy expended, heart rate, pulse rate, speed, stride length, resistance, sleep durations, and target values. It also includes settings such as age, body height, body weight, maximum recommended heart rate, sustained physical activity threshold, and TAT threshold.
For implementers, this means the device specialization is not limited to steps or heart rate alone. It covers the data needed for exercise tracking, training targets, sleep reporting, and derived calculations that use user settings and observed activity.
The domain information model is tightly defined
Clause 6 defines the cardiovascular fitness and activity monitor domain information model. The model uses the medical device system (MDS) object and the standard object families for numeric objects, RT-SA objects, enumeration objects, PM-store objects, and scanner objects.
The clause also defines attribute qualifiers and their meaning. Mandatory, conditional, recommended, not recommended, and optional attributes tell implementers what must be present, what depends on conditions, and what should be avoided, while static, dynamic, and observational attributes define when values may change and when those values have to be reported for correct interpretation.
Extensions are limited and vendor-specific codes are controlled
Clause 6 states that no class extensions are defined with respect to ISO/IEEE 11073-20601. If a vendor needs extra metrics or attributes, the standard allows vendor-specific extension, but those items must use private numbering space codes in the range reserved for that purpose.
That rule matters because it preserves interoperability. Devices can extend the model when needed, but private additions stay clearly separated from the standardized nomenclature so managers can distinguish standard data from vendor-specific data.
The service model uses GET, SET, event reports, and actions
Clause 7 defines the service model used for data exchange. The supported services include GET for retrieving object attribute values, SET for changing object values where allowed, event report service for configuration and measurement reports, and action service for invoking methods such as Set-Time.
The document also states that event reports are used in confirmed mode and that agent-initiated measurement transmission shall be supported. Managers shall support both single-person and multiple-person event reports, while agents may support one or both styles depending on the deployment.
The communication model fixes the association and operating behavior
Clause 8 describes the association procedure, the configuring procedure, the operating procedure, and time synchronization. If an agent receives an association response of accepted-unknown-config, it enters the Configuring state and follows the 20601 configuration procedure.
Time synchronization is allowed, but the synchronization mechanism itself is outside the scope of this standard. If time synchronization is used, the agent reports that fact in the Mds-Time-Info attribute so the manager can interpret time-based observations correctly.
Conformance is stated through levels and ICS
Clause 10 says the standard is used with ISO/IEEE 11073-20601 and supports conformance for the domain information model, nomenclature code values, protocol and service models, and the communication service model. It defines conformance levels, including base conformance and extended nomenclature, and requires vendors to provide implementation conformance statements (ICS).
In practice, this is the part a buyer or quality team checks when comparing products. It tells you how a supplier declares which objects, attributes, events, and code sets are implemented and whether any extensions are used.
The standard includes two device profiles
Clauses 11 and 12 define the step counter profile and the activity monitor profile. The step counter profile centers on distance in steps, meters, or feet and may also include speed, stride length, cadence, energy expended, estimated weight loss, power, and subsessions.
The activity monitor profile covers kinematic data such as acceleration, speed, position, and orientation and requires support for continuous monitoring sessions and scanners. That difference matters when deciding whether a device is intended mainly for step-based summaries or for richer continuous activity tracking.
What terms does ISO/IEEE FDIS 11073-10441 define?
- Personal health device (PHD) - A device used in personal health applications.
- Agent - A node that collects personal health data and transmits it to an associated manager.
- Manager - A node that receives data from one or more agent systems, such as a phone, health appliance, set top box, or computer.
- Session - The envelope that contains all measurements associated with one activity or exercise episode.
- Subsession - A portion of a session that contains measurements for a separate phase or segment of the activity.
- Real-time sample array (RT-SA) - A model for transmitting high-rate sample sequences, used here for periodic sampling of waveform-like data.
- Proportional integral mode (PIM) - The integral of a variable over the duration of a session or subsession, used in actigraphy.
- Time above threshold (TAT) - The total time that a variable exceeds a given threshold during a session or subsession.
Who uses ISO/IEEE FDIS 11073-10441?
ISO/IEEE FDIS 11073-10441 is used by manufacturers and developers of personal telehealth cardiovascular fitness and activity monitor devices, including treadmills, exercise bikes, heart rate monitors, bike computers, pedometers, watches, bracelets, and rings. It is also used by developers of managers such as cell phones, personal computers, personal health appliances, and set top boxes that receive and interpret the device data.
Quality teams, interoperability testers, and buyers use it to check the supported objects, services, conformance level, and supported profiles. Regulatory and integration teams use it to map the device’s measurements and reports into a shared ISO/IEEE 11073 communication model.
What changed in ISO/IEEE FDIS 11073-10441 from the previous edition?
ISO/IEEE FDIS 11073-10441 is the second edition and replaces ISO/IEEE 11073-10441:2015. The document says it is technically revised, and it lists these main changes:
- updated the normative reference to ISO/IEEE 11073-20601-2019
- updated the device specialization version and the association details
- revised the wording in 6.3 about observational attributes
- added an ICS statement for the device specialization version
- expanded the DIM extensibility rule in 6.12
- updated 8.5.2 for attribute-id-list compliance with 20601-V4
- removed the year in the bibliography so the latest version applies
- made ISO/IEEE 11073-10101 a normative reference
- clarified nomenclature precedence between ISO/IEEE 11073-10101, ISO/IEEE 11073-20601, 104zz, and this standard
- tied nomenclature-version to protocol-version
- corrected some nomenclature code values
- added target-range concepts for heart rate, speed, and energy expended
- added the concepts of user exercise target and health status
Which standards are used with ISO/IEEE FDIS 11073-10441?
ISO/IEEE FDIS 11073-10441 is used with ISO/IEEE 11073-20601, which provides the optimized exchange protocol, the core object and service model, and the encoding rules that this specialization builds on. It also uses ISO/IEEE 11073-10101 for nomenclature, so the object and attribute codes have a shared meaning.
The context also says that ISO/IEEE 11073-20601 draws information from ISO/IEEE 11073-10201 and ISO/IEEE 11073-20101. In practice, that means this specialization sits inside the wider 11073 family and depends on the shared 20601 model rather than redefining it.
What does the ISO/IEEE FDIS 11073-10441 document contain?
The document contains measurement concepts and formulas in Clause 5, including activity intensity, heart rate targets, maximum recommended heart rate, PIM, RMS, and TAT. It also contains the domain information model in Clause 6, with object instance diagrams, attribute tables, configuration types, scanner objects, PM-store objects, and extensibility rules.
Clause 7 contains the service model and the supported object access and event reporting services, while Clause 8 describes association, configuring, operating, and time synchronization procedures. Clauses 10 to 12 cover conformance and the step counter and activity monitor profiles, and the annexes provide ASN.1 definitions, nomenclature code allocations, and revision history.

Updating of IEEE Standards documents
Users of IEEE Standards documents should be aware that these documents may be superseded at any time
by the issuance of new editions or may be amended from time to time through the issuance of amendments,
corrigenda, or errata. An official IEEE document at any point in time consists of the current edition of the
document together with any amendments, corrigenda, or errata then in effect.
Every IEEE standard is subjected to review at least every 10 years. When a document is more than 10 years
old and has not undergone a revision process, it is reasonable to conclude that its contents, although still of
some value, do not wholly reflect the present state of the art. Users are cautioned to check to determine that
they have the latest edition of any IEEE standard.
In order to determine whether a given document is the current edition and whether it has been amended
through the issuance of amendments, corrigenda, or errata, visit IEEE Xplore or contact IEEE. For more
information about the IEEE SA or IEEE’s standards development process, visit the IEEE SA Website.
Errata
Errata, if any, for all IEEE standards can be accessed on the IEEE SA Website. Search for standard number
and year of approval to access the web page of the published standard. Errata links are located under the
Additional Resources Details section. Errata are also available in IEEE Xplore. Users are encouraged to
periodically check for errata.

Patents
IEEE standards are developed in compliance with the IEEE SA Patent Policy.
Attention is called to the possibility that implementation of this standard may require use of subject matter
covered by patent rights. By publication of this standard, no position is taken by the IEEE with respect to the
existence or validity of any patent rights in connection therewith. If a patent holder or patent applicant has
filed a statement of assurance via an Accepted Letter of Assurance, then the statement is listed on the IEEE
SA Website at https://standards.ieee.org/about/sasb/patcom/patents.html. Letters of Assurance may indicate
whether the Submitter is willing or unwilling to grant licenses under patent rights without compensation or
under reasonable rates, with reasonable terms and conditions that are demonstrably free of any unfair
discrimination to applicants desiring to obtain such licenses.
Essential Patent Claims may exist for which a Letter of Assurance has not been received. The IEEE is not
responsible for identifying Essential Patent Claims for which a license may be required, for conducting
inquiries into the legal validity or scope of Patents Claims, or determining whether any licensing terms or
conditions provided in connection with submission of a Letter of Assurance, if any, or in any licensing
agreements are reasonable or non-discriminatory. Users of this standard are expressly advised that
determination of the validity of any patent rights, and the risk of infringement of such rights, is entirely their
own responsibility. Further information may be obtained from the IEEE Standards Association.
IMPORTANT NOTICE
Technologies, application of technologies, and recommended procedures in various industries evolve over
time. The IEEE standards development process allows participants to review developments in industries,
technologies, and practices, and to determine what, if any, updates should be made to the IEEE standard.
During this evolution, the technologies and recommendations in IEEE standards may be implemented in
ways not foreseen during the standard’s development. IEEE standards development activities consider
research and information presented to the standards development group in developing any safety
recommendations. Other information about safety practices, changes in technology or technology
implementation, or impact by peripheral systems also may be pertinent to safety considerations during
implementation of the standard. Implementers and users of IEEE Standards documents are responsible for
determining and complying with all appropriate safety, security, environmental, health, data privacy, and
interference protection practices and all applicable laws and regulations.

Introduction
This introduction is not part of IEEE Std 11073-10441-2025, Health Informatics—Device Interoperability—Part 10441:
Personal Health Device Communication—Device Specialization—Cardiovascular Fitness and Activity Monitor.
Within the context of the ISO/IEEE 11073 family of standards for device communication, a normative
definition of communication between personal telehealth cardiovascular fitness and activity monitor devices
and managers (e.g., cell phones, personal computers, personal health appliances, and set top boxes) is
established in this standard in a manner that enables plug-and-play interoperability. Appropriate portions of
existing standards are leveraged including ISO/IEEE 11073 terminology, information models, application
profile standards, and transport standards. The use of specific term codes, formats, and behaviors in telehealth
environments restricting optionality in base frameworks in favor of interoperability is specified. A common
core of communication functionality for personal telehealth cardiovascular fitness and activity monitor
devices is defined in this standard.

Avant-propos
(voir www.iso.org/directives).

https://standards.ieee.org/about/sasb/patcom/patents.html.

Introduction

forme cardiovasculaire
1. Description générale
1.1 Domaine d'application

L'utilisation de "will" est déconseillée en anglais et il ne peut pas être utilisé lors de l'établissement d'exigences obligatoires;
